不同程度阻塞性睡眠呼吸暂停低通气综合征妊娠结局的影响

摘　　要：目的:探讨不同程度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)对孕妇妊娠结局的影响。方法:选取2016年1月-2017年6月本院收治的156例阻塞性OSAHS妊娠期患者以及100例正常妊娠孕妇为研究对象。根据疾病的严重程度分为A组(轻度OSAHS)100例、B组(中、重度OSAHS)56例,正常妊娠孕妇为C组100例。比较三组血压、妊娠结局及新生儿情况。结果:A、B组孕妇收缩压、舒张压均高于C组,终止妊娠孕周均小于C组,且B组孕妇收缩压、舒张压均高于A组,终止妊娠孕周小于A组,比较差异均有统计学意义(P<0.05);C组顺产率均高于A、B组,剖宫产率及引产率均低于A、B组,比较差异均有统计学意义(P<0.05);A、B组顺产率比较差异无统计学意义(P>0.05),但A组剖宫产率高于B组,引产率低于B组,比较差异均有统计学意义(P<0.05);B组孕妇并发症发生率和终止妊娠指征占比率均高于A组,比较差异均有统计学意义(P<0.05);C组新生儿5 min阿氏评分、体重均高于A、B组,且A组均高于B组,比较差异均有统计学意义(P<0.05);且C组新生儿转科率均低于A、B组,且A组低于B组,比较差异均有统计学意义(P<0.05)。结论:中、重度OSAHS患者的母婴结局显著劣于轻度OSAHS患者,劣于正常妊娠孕妇,可为后续不同程度OSAHS患者选择不同的治疗方法的研究提供理论基础。Objective:To investigate the effect of obstructive sleep apnea hypopnea syndrome(OSAHS)on pregnancy outcome in pregnant women.Method:200 pregnant women with obstructive sleep apnea hypopnea syndrome and 100 normal pregnant women from January 2016 to June 2017 in our hospital were selected.According to the severity of the disease,they were divided into 100 cases of group A(mild OSAHS),56 cases of group B(moderate and severe OSAHS),100 cases of group C(normal pregnant women).The blood pressure,pregnancy outcome and newborn among the three groups were compared.Result:The systolic blood pressure and diastolic blood pressure of pregnant women in group A and B were all higher than those of group C,the gestational weeks of termination of pregnancy were lower than those of group C,and the systolic blood pressure and diastolic blood pressure of pregnant women in group B were higher than those of group A,the gestational weeks after termination of pregnancy were lower than those of group A,the differences were statistically significant(P<0.05).The spontaneous delivery rate in group C was higher than those of group A and B,and the rate of cesarean section and induced labor were all lower than those of group A and B,the differences were statistically significant(P<0.05),the spontaneous delivery rate in group A and B were compared,the difference was not statistically significant(P>0.05),but the cesarean section rate in group A was higher than that of group B,the induced labor rate was lower than that of group B,the differences were statistically significant(P<0.05).The incidence of complications and termination of pregnancy in group B were higher than those of group A(P<0.05).The Apgar scores of 5 min of newborn and body weight in group C were higher than those of group A and B,and group A was higher than that of group B,the differences were statistically significant(P<0.05),the neonatal transfer rates of group C were lower than those of group A and B,and group A was lower than that of group B,the differences were statisticall
