摘 要:目的:观察无创双水平正压通气(BiPAP)治疗对重症支气管哮喘患者肺功能及动脉血气的影响。方法:选取本院收治的84例重症支气管哮喘患者,按照治疗方法不同将其分为对照组(n=40)和观察组(n=44)。对照组给予常规对症支持治疗,观察组在对照组基础上予以BiPAP治疗。比较两组治疗效果、治疗前后的动脉血气指标(pH、PaCO_2、PaO_2、SaO_2)及肺功能指标(FEV_1、FEV_1/FVC、PEF)变化。结果:观察组治疗总有效率为95.45%,明显高于对照组的75.00%,比较差异有统计学意义(X^2=4.761,P=0.027);治疗后,两组pH、PaO_2、SaO_2均高于治疗前,PaCO_2低于治疗前,且观察组pH、PaO_2、SaO_2均高于对照组,PaCO_2低于对照组,比较差异均有统计学意义(P<0.05);治疗后,两组FEV_1、FEV_1/FVC、PEF均高于治疗前,且观察组均高于对照组,比较差异均有统计学意义(P<0.05)。结论:BiPAP可明显改善重症支气管哮喘患者肺功能及动脉血气指标,显著性提高临床疗效。Objective:To observe the effect of noninvasive bi-level positive airway pressure(BiPAP)on pulmonary function and arterial blood gas of patients with severe bronchial asthma.Method:A total of 84 patients with severe bronchial asthma in our hospital were selected.According to different treatment methods,they were divided into control group(n=40)and observation group(n=44).The control group received routine symptomatic supportive treatment,while observation group received BiPAP treatment on the basis of control group.The therapeutic effect,arterial blood gas index(pH,PaCO2,PaO2,SaO2)and pulmonary function index(FEV1,FEV1/FVC,PEF)before and after treatment between two groups were compared.Result:The total effective rate of observation group was 95.45%,which was significantly higher than 75.00%of control group,the difference was statistically significant(χ2=4.761,P=0.027).After treatment,the pH,PaO2 and SaO2 of two groups were higher than those of before treatment,and PaCO2 was lower than those of before treatment,the pH,PaO2 and SaO2 of observation group were higher than those of control group,and PaCO2 was lower than that of control group,the differences were statistically significant(P<0.05).After treatment,the FEV1,FEV1/FVC and PEF in two groups were all higher than those of before treatment,and observation group were higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:BiPAP can significantly improve the pulmonary function and arterial blood gas indexes of severe bronchial asthma patients,and significantly improve the clinical efficacy.
