主动电极右心室间隔部起搏损伤电流、起搏参数和电极稳定性的关系

摘　　要：目的探讨主动电极右心室间隔部(RVS)起搏损伤电流(COI)、起搏阈值、R波感知阈值、电极阻抗和被植入电极稳定性的关系。方法选取主动电极RVS起搏患者82例,术中或术后根据电极是否发生脱位分为A组(无脱位组)76例和B组(脱位组)6例,分别测试电极固定即刻损伤电流(COI0 min)和10 min后损伤电流(COI10 min),测试电极固定术后10 min和3个月后起搏阈值、R波感知阈值和电极阻抗。结果术中A组COI0 min、COI10 min均高于B组,两组比较差异均有统计学意义(P <0.01);电极固定10 min后测试A组起搏阈值低于B组,A组R波感知阈值高于B组,两组比较差异均有统计学意义(P <0.01)。结论主动电极RVS起搏植入术中损伤电流大、起搏阈值低、R波感知阈值高提示主动电极固定的稳定性好。Objective To explore the relationship among current of injury(COI),pacing threshold,R wave perception threshold,electrode impedance and the stability of the implanted electrodes.Methods We recruited 82 patients with right ventricular septum pacing.Based on the condition whether or not the electrode was dislocated during or after the operation,76 cases of no dislocation were selected into group A and 6 cases with dislocation were selected into group B.During the process of electrode fixation,we measured separately the immedi-ate current of injury(COI0 min)and the 10 minutes current of injury(COI10 min).When the electrode was fixed 10 minutes and after three months,we measured separately the pacing threshold,the R wave perception threshold and the electrode impedance.Results COI0 min and of the group A were respectively higher than group B.The differ-ence of the two groups was statistically significant(P<0.01).After the electrode was fixed for 10 min,the pacing threshold of group A was lower than that of group B.The R wave perception threshold of the A group was higher than that of the B group.The difference of the two groups was statistically significant(P<0.01).Conclusion It is suggested that the stability of the active electrode is good if the current of injury is large.The threshold of pacing is low and the perception threshold of R wave is high.
