摘　　要：目的探讨结核感染T细胞斑点试验(T-SPOT.TB)在结核病诊断中的价值及其影响因素。方法收集我院2015年1月至2017年5月住院期间的疑似结核病患者325例,分别进行T-SPOT.TB、结核菌素试验(PPD)、血清结核抗体(TB-Ab)检测,探讨三者在结核病诊断中的价值,并分析性别、年龄、血常规及白蛋白对T-SPOT.TB的影响。结果 T-SPOT.TB、PPD、TB-Ab三种检测方法在结核病诊断方面,阳性率明显高于非结核病,同时,T-SPOT.TB对于结核病的诊断阳性率又明显高于另两种方法(P <0.05)。淋巴细胞绝对值与T-SPOT.TB呈正相关,NLR(中性粒细胞绝对值/淋巴细胞绝对值)与T-SPOT.TB呈负相关,PLR(血小板计数/淋巴细胞绝对值)与T-SPOT.TB呈负相关,差异有统计学意义(P <0.05)。结论 T-SPOT.TB对于诊断结核病的敏感度和特异性较高,在结核病诊断中有较好的辅助价值,淋巴细胞绝对值、NLR及PLR对T-SPOT.TB有显著影响。Objective To explore the value of t-spot test(T-SPOT.TB)in the diagnosis of tuberculosis and its influencing factors.Methods The study involved,from January 2015 to May 2017,325 hospitalized patients with suspected tuberculosis(TB).T-SPOT.TB,tuberculin skin test(PPD)and serum tuberculosis anti-body(TB-Ab)detection were conducted and the value of the three in the diagnosis of tuberculosis was explored.The influence of gender,age,blood routine and albumin on T-SPOT.TB was analyzed.Results The positive rate of T-SPOT.TB,PPD and TB-Ab in TB patients was significantly higher than that in non-TB patients,and the posi-tive rate of T.POT.TB for TB diagnosis was significantly higher than that of the other two methods(P<0.05).The absolute value of lymphocytes was positively correlated with T-SPOT.TB but NLR(neutrophil absolute value/lym-phocyte absolute value)and PLR(platelet count/lymphocyte absolute value)negatively with T.SPOT.TB(P<0.05).Conclusions T-SPOT.TB has a high sensitivity and specificity in the diagnosis of TB,and has a good auxiliary value in the diagnosis of TB.The absolute value of lymphocytes,NLR and PLR has a significant impact on T.SPOT.TB.
