摘 要:目的探讨不同灌注压力条件下,经腹主动脉腔内灌注低浓度弹力蛋白酶对兔腹主动脉瘤(AAA)成瘤率及死亡率的影响。方法 30只雄性新西兰大白兔随机分为A、B、C三组,每组各10只,均经双腔导管向腹主动脉腔内注入10μl浓度为50 u/ml的弹力蛋白酶,灌注保持时间为7~10 min,灌注保持压力分别为A组0 mmHg,B组100~200 mmHg,C组300~400 mmHg,分别于术前及术后第7天行腹主动脉超声检查,计算各组腹主动脉扩张率、成瘤率及存活率,后处死动物,对血管灌注部位行组织病理及免疫组织化学染色。结果将灌注后血管直径大于正常腹主动脉内经的50%者定义为腹主动脉瘤形成。A组存活10只,灌注部位血管扩张率为(16±9.4)%,成瘤率为0%,存活率为100%;B组存活10只,灌注部位血管扩张率为(98.8±16.4)%,成瘤率100%,存活率为100%;C组存活4只,存活动物灌注部位血管扩张率为(110±13.3)%,成瘤率100%,存活率为40%。结论经腹主动脉腔内灌注低浓度弹力蛋白酶(50 u/ml),将灌注压保持在100~200 mmHg,能够在较短时间内成功制备腹主动脉瘤模型,且成瘤率高、死亡率低。Objective To explore the rabbit abdominal aortic aneurysm model by abdominal aortic perfusion with low concentration,and observe the effect of different perfusion pressure on the abdominal aorta aneurysm formation rate and mortality in rabbits.Methods 30 male New Zealand rabbits were randomly divided into group A,B,C,with 10 rabbits in each group.10 ul of 50 U/ml elastase was perfused into the abdominal aorta in each rabbit by a doublelumen catheter,perfusion retention time were 7 to 10 minutes,perfusion retention pressure was:0 mmHg for group A,100 to 200 mmHg for group B and 300 to 400 mmHg for group C,and then the doppler ultrasound examinations of the abdominal aorta were performed before surgery and on the seventh day after surgery.The abdominal aorta dilation rate,abdominal aorta aneurysms formation rate and survival rate were calculated,and then the rabbits were sacrificed.The perfused segment of the abdominal aorta was performed histopathologic and immunohistochemical staining.Results An abdominal aortic aneurysm formation is defined as a blood vessel with a diameter greater than 50%of the normal abdominal aorta after perfusion.10 rabbits in group A survived,the perfusion segment dilation rate was 16±9.4%,the aneurysm formation rate was 0%,and the survival rate was 100%.10 rabbits in group B survived the perfusion segment dilation rate was 98.8±16.4%,the aneurysm formation rate was 100%and the survival rate was 100%.Therefore,only 4 rabbits in group C survived,the perfusion segment dilation rate was 110±13.3%,the aneurysm formation rate was 100%and the survival rate was 40%.Conclusions Abdominal aortic perfusion of low concentrations of elastase,perfusion retention pressure 100 to 200 mmHg is feasible method to induce AAA in rabbits with a short perfusion time,high aneurysm formation rate and low death rate.
