摘　　要：探讨慢病防控在精准扶贫地区存在的问题,提出可供参考的对策、方案,为健康扶贫制度的完善和推广提供理论依据,更好地解决"因病致贫、因病返贫"的问题。通过实地考察、定性访谈、问卷调查等方式,分析了本市现阶段精准扶贫地区慢病防控的现状及存在的问题,并提出了把慢病管理控制在基层、提升基层医疗服务能力、实施家庭医疗团队签约服务等对策和建议,以期待我市精准扶贫地区慢病防控能持续有效得到控制。To explore the problems existing in the prevention and control of chronic diseases in the areas of precision poverty alleviation,and propose countermeasures for reference,providing a theoretical basis for the improvement and promotion of the health poverty alleviation system,and better solve the problem of"poorness due to illness and returning to poverty due to illness".Through the field examination,qualitative interviews,questionnaires and other means,the current situation and problems of chronic disease prevention and control in the precise poverty alleviation areas of our city were analyzed,and put forward countermeasures and suggestions for controlling chronic disease management at the grassroots level,improving primary medical service capabilities,and implementing family medical team signing services,in view of the expectation that the prevention and control of chronic diseases in the precise poverty alleviation areas of our city can be effectively controlled.
