一例2型糖尿病患者24 h尿蛋白定量结果的分析

摘　　要：目的探讨24 h尿蛋白定量结果的临床判读。方法通过结合尿常规、尿沉渣检测和尿蛋白电泳结果,分析一例2型糖尿病患者在留置导尿管和未留置导尿管期间24 h尿蛋白定量结果的变化。结果患者24 h尿蛋白检测结果的变化与留置导尿管没有必然联系。该患者24 h尿蛋白检测结果阳性时,尿蛋白电泳显示尿蛋白成分复杂,多次检出α2巨球蛋白,提示患者尿蛋白来自于肾后性疾病;尿常规和尿沉渣检测结果显示存在大量细菌和红白细胞,提示患者存在感染,导致尿蛋白定量结果假阳性。结论解读24 h尿蛋白定量结果时,应注意结合尿常规、尿沉渣检测和尿蛋白电泳结果具体判断尿蛋白的来源。Objective To explore the interpretation of 24 hours urinary protein.Methods By combination of urine routine,urine sediment and urine protein electrophoresis,the results of 24 hours urinary protein in a type 2 diabetic mellitus(DM2)patient were analyzed in the condition with and without a catheter.Results There s no association between 24 hours urinary protein and catheter indwelling.When 24 hours urinary protein was positive,urine electrophoresis showed the components of urine protein are complex and presentα2 macroglobulin suggested the source of urine protein from post-renal diseases.In the urine routine and urine sediment tests lots of bacteria,blood cells and white cells were detected,which implied the patients suffering with urinary infection and caused the false positive results of 24 hours urine protein.Conclusions When explaning the results of 24 hours urine protein,clinicians will combine urine routine,urine sediment and urine protein electrophoresis to document a comprehensive judgement for the source of urine protein.
