延迟断脐对第三产程及产后2小时阴道出血量的影响

摘　　要：目的探讨延迟断脐对产妇第三产程时间及产后2 h阴道出血量的影响。方法选取2017年9月—11月在北京市某三级甲等专科医院进行自然分娩的产妇,按数字随机分组法分为实验组(52例)和对照组(42例)。实验组采用延迟断脐,即待脐带停止搏动后断脐。对照组按照常规断脐。两组产妇均为健康单胎、无高危因素及非正常胎儿者。产妇年龄25～35岁,孕周37～41周。分析两组产妇第三产程时间及产后2 h阴道出血量。结果实验组年龄(31. 1±3. 1)岁,孕周(39. 2±1. 0)周;对照组年龄(30. 3±3. 2)岁,孕周(39. 2±1. 1)周。实验组产妇第三产程时间为(8. 6±6. 2) min,对照组第三产程时间为(6. 7±2. 3) min,两组差异无统计学意义;实验组产妇产后2 h阴道出血量为(271. 2±140. 2) ml,对照组产妇为(395. 1±188. 8) ml,实验组产妇产后2 h阴道出血量明显低于对照组,两组差异有统计学意义。结论延迟断脐对产妇第三产程时间无影响,但可减少产后2 h阴道出血量。Objective To investigate the effect of delayed cord clamping on the length of third stage of labor and postpartum hemorrhage within 2 hours after delivery.Methods Women who gave birth with natural delivery at a Class Three Group A special hospital were enrolled in Beijing from September 2017 to November 2017.Two groups were randomly divided with 52 cases in experimental group and 42 cases in control group.In the experimental group,the delayed cord clamping was conducted,in which the clamping was operated after stopping the pulsation of the umbilical cord,while the clamping was conducted in regular way in the control group.The length of third stage of labor and postpartum hemorrhage within 2 hours after delivery was recorded and compared between the two groups.The two groups were both healthy singletons,no high-risk factors and no abnormal fetuses.The maternal ages were 25~35 years.The gestational ages were 37~41 weeks.Results The materal ages was(31.1±3.1)years old and the gestational age was(39.2±1.0)weeks in the experimental group.The materal ages was(30.3±3.2)years old and the gestational age was(39.2±1.1)weeks in the control group.The length of third stage of labor was(8.6±6.2)min and the amount of postpartum hemorrhage within 2 hours was(271.2±140.2)ml in the experimental group,while it was(6.7±2.3)min and(395.1±188.8)ml in the control group respectively.There was no significant difference on the length of third stage of labor between the two groups,while the difference of postpartum hemorrhage between the two groups was statistically significant,with lower amount of postpartum hemorrhage in the experimental group.Conclusion Delayed cord clamping did not affect the length of the third stage of labor,but it could reduce the amount of postpartum hemorrhage within 2 hours.
