摘  要:目的:了解医院感染性铜绿假单孢菌临床分布特征及耐药谱的变迁,为抗菌药物的使用提供参考依据。方法:收集2013年1月-2016年12月共2017株铜绿假单孢菌进行药敏试验,采用WHONET 5.6软件进行数据分析。结果:2013-2016年的4年里铜绿假单孢菌均为医院感染性致病菌的第二位,特别在痰标本中占致病菌的首位。阿米卡星敏感率维持在90%以上,而头孢替坦、复方新诺明、头孢曲松、氨苄西林/舒巴坦、呋喃妥因、头孢唑啉耐药率维持在90%以上。对妥布霉素及庆大霉素耐药率有所下降,而对亚胺培南耐药率则有明显上升。结论:致病性铜绿假单孢菌的重要性和耐药性正越来越被重视。治疗首选阿米卡星,其次是氨基糖苷类和喹诺酮类。除非药敏试验支持,一般不考虑使用头孢替坦、复方新诺明、头孢曲松、氨苄西林/舒巴坦、呋喃妥因、头孢唑啉。Objective:To investigate the clinical distribution characteristics and resistance spectrum of Pseudomonas aeruginosa,and to provide reference for the application of antimicrobial agents.Methods:2017 strains of Pseudomonas aeruginosa were tested for susceptibility testing from January 2013 to December 2016,and data were analyzed by WHONET5.6 software.Results:Pseudomonas aeruginosa was the second place in the hospital infection pathogens in the four years from 2013 to 2016,especially in the sputum specimens was the first place.The sensitivity of amikacin was maintained at over 90%,while the resistance of cefotetine,cotrimoxazole,cefotroxol,ampicillin/sulbactam,nitrofurantoin and cefazolin were maintained at over 90%.The resistance rate to tobramycin and gentamicin were decreased,while the resistance rate to imipenem was increased significantly.Conclusion:The importance and resistance of Pseudomonas aeruginosa are becoming more and more important.Amikacin was preferred,followed by aminoglycosides and quinolones.Unless the susceptibility testing is supported,generally do not consider the application of cefotetine,cotrimoxazole,ceftriaxone,ampicillin/sulbactam,nitrofurantoin and cefazolin.
