内括约肌侧切术与后正位切除术治疗陈旧性肛裂的临床疗效比较

摘　　要：目的对比内括约肌侧切术与后正位切除术治疗陈旧性肛裂的临床效果。方法选取2017年3月至2018年4月在揭阳市中医院就诊的陈旧性肛裂患者80例,根据随机数表法分为两组,每组40例。对照组采用后正位切除术治疗,研究组予内括约肌侧切术治疗,对比两组术中、术后相关指标、疗效及并发症发生情况。结果研究组手术时间、伤口愈合时间及肛门疼痛消失时间均短于对照组,术后视觉模拟评分法(VAS)评分低分对照组,差异有统计学意义(P<0.05);研究组治疗总有效率及并发症发生率与对照组对比,差异无统计学意义(P>0.05)。结论内括约肌侧切术与后正位切除术治疗陈旧性肛裂均有一定的治疗效果,而内括约肌侧切术具有创面愈合快、疼痛轻的特点。Objective To compare the clinical effects of lateral incision of internal sphincter and posterior incision in treatment of old anal fi ssure.Methods Eighty patients with old anal fi ssure in the hospital from March 2017 to April 2018 were selected,and they were divided into two groups by random number table,with 40 cases in each group.Control group was given posterior incision,while research group was treated with lateral incision of internal sphincter.The intraoperative and postoperative indexes,efficacy,complications were compared between the two groups.Results The operation time,wound healing time,disappearance time of anal pain in research group were shorter than those in control group,and the postoperative visual analogue scale(VAS)in observation group was lower than control group(P<0.05);There was no statistical difference in the total effective rate and incidence of complications between the two groups(P>0.05).Conclusion Lateral incision of internal sphincter and posterior incision in treatment of old anal fi ssure are effective,but lateral incision of internal sphincter has features of quick wound healing and light pain.
