鞘内注射牛痘疫苗接种家兔炎症皮肤提取物对腰椎间盘突出大鼠的抗炎镇痛作用

摘　　要：目的:观察牛痘疫苗接种家兔炎症皮肤提取物(神经妥乐平neurotropin, NTP)对腰椎间盘突出(lumbar disc herniation, LDH)大鼠的抗炎镇痛作用。方法:大鼠随机分为LDH+NTP组、Sham组及LDH+Saline组。测定不同时间点大鼠左后足底50%机械缩足阈值(paw withdrawal mechanical threshold, PWT),用Western-blot法检测术侧L3-5背根神经节(dorsal root ganglion, DRG)炎症细胞因子的表达。结果:(1)与术前1 d相比,Sham组术后各时间点PWT无明显降低,提示其对机械刺激未产生明显的痛觉过敏;LDH+NTP组与LDH+Saline组术后各时间点PWT均明显降低,与Sham组相比差异均有统计学意义(P<0.05),提示其对机械刺激产生了明显的痛觉过敏;LDH+NTP组鞘内给药后各时间点PWT高于LDH+Saline组,差异有统计学意义(P<0.05),提示鞘内注射NTP可缓解LDH大鼠的痛觉过敏;(2)用药7 d后,LDH+NTP组炎症细胞因子IL-1β、IL-6、TNF-α的表达均较LDH+Saline组明显降低(P<0.05),但显著高于Sham组(P<0.05)。结论:鞘内注射NTP可明显缓解LDH大鼠神经根性疼痛,下调DRG中炎症细胞因子的表达,抑制神经根的炎性反应,产生抗炎镇痛作用。Objective:To investigate the anti-inflammatory and analgesic effects of intrathecal neurotropin(NTP)in rats with lumbar disc herniation(LDH).Methods:Rats were randomly divided into three groups:LDH+NTP,Sham and LDH+Saline groups.Paw withdrawal mechanical thresholds(PWT)were observed at different time points.L3-5 dorsal root ganglion(DRG)on the operative side was collected,and Western-blot method was used to detect the expression of the inflammatory cytokines.Results:(1)Compared with day 1 before operation,there was no significant decrease in PWT at all time points in the Sham group,and in LDH+NTP group and LDH+Saline group,PWT decreased significantly at all time points.Compared with Sham group,the PWTs in LDH+NTP and LDH+Saline groups were significanly decreased after operation(P<0.05);At day 1,3,and 7 after the first NTP/Saline injection,compared with the LDH+Saline group,the PWTs in LDH+NTP group were significantly increased(P<0.05).(2)Seven days after intrathecal injection of NTP,the expression of inflammatory cytokines in L3-5 DRG in the LDH+NTP group were significantly decreased than those in the LDH+Saline group(P<0.05),but significantly higher than those in the Sham group(P<0.05).Conclusion:Intrathecal injection of neurotropin can significantly alleviate the neuropathic pain in rats with LDH,downregulate the expression of inflammatory cytokines in DRG,inhibit the inflammatory response of nerve roots,and produce anti-inflammatory and analgesic effects.
