产后腰骶臀腿肌筋膜综合征的相关因素分析

摘　　要：目的:对产后腰骶臀腿肌筋膜综合征(postpartum lumbar, sacral, gluteal and leg myofascial syndrome, P-LSGL-MS)的高危因素进行分析,为疾病的诊断和防治提供参考。方法:回顾性分析2015年6月至2017年5月在我院分娩产妇的临床资料,筛选出598例为研究对象,对研究对象的年龄、HLA-B27、产次、孕周、分娩方式、妊娠中后期有无腰骶臀腿痛及既往有无腰骶臀腿痛病史与产后腰骶臀腿肌筋膜综合征发病的相关性进行分析,探讨产后腰骶臀腿肌筋膜综合征发病的高危因素。结果:598例产妇中共有192例发生产后腰骶臀腿肌筋膜综合征。通过单因素及多因素Logistic回归分析显示,HLA-B27阳性(OR=1.589, P=0.033)、妊娠中后期有腰骶臀腿痛(OR=1.437, P=0.042)及既往有腰骶臀腿痛病史(OR=1.481, P=0.036),均为导致孕妇生产后出现产后腰骶臀腿肌筋膜综合征的独立危险因素。结论:HLA-B27阳性、妊娠中后期有腰骶臀腿痛及既往有腰骶臀腿痛病史为产后腰骶臀腿肌筋膜综合征的高危因素,对此类产妇临床应采取预防措施。Objective:To analyze the high-risk factors of postpartum lumbar,sacral,gluteal and leg myofascial syndrome(P-LSGL-MS),so as to provide references for the diagnosis and treatment for the disease.Methods:Clinical Data of women who delivered in our hospital from June 2015 to May 2017 were figured out retrospectively.598 cases were selected as research subjects.In order to identify high risk factors of P-LSGL-MS,univariate and multivariate logistic regression analysis were employed for the analysis of the relationship between the age,HLA-B27,parity,gestational week,mode of delivery,the waist-hip pain during the middle and late period of gestation,the history of waist-hip pain and the incidence of P-LSGL-MS.Results:In this retrospective study of 598 cases,a total of 192 cases suffered from P-LSGL-MS.Univariate and multivariate logistic regression analysis showed that HLA-B27 positive(OR=1.589,P=0.033),the waist-hip pain during the middle and late period of gestation(OR=1.437,P=0.042)and with the history of waist-hip pain(OR=1.481,P=0.036)were the high risk factors for P-LSGL-MS.Conclusion:HLA-B27,the waist-hip pain during the middle and late gestation period,and history of waist-hip pain are the highrisk factors of P-LSGL-MS,and corresponding preventive actions should be taken in the clinic.
