胸腔镜辅助小切口治疗创伤性连枷胸52例临床分析

摘　　要：目的探讨胸腔镜辅助小切口治疗创伤性连枷胸的临床效果。方法该研究方便选取该院胸外科2010年4月—2017年10月收治的52例创伤性连枷胸患者,以胸腔镜辅助小切口处理胸腔内损伤,包括清除积血、探查止血、行肺裂伤修补手术等,然后处理肋骨骨折,依据胸腔镜下所观察到的肋骨骨折部位、数目等行肋骨内固定。观察手术时间、住院时间、术后引流量及术后并发症。结果 52例连枷胸患者平均手术时间(70.9±4.6)min,平均住院时间(13.4±5.3)d,术后平均引流量(289.2±9.8)mL。全组病例未出现严重并发症。结论胸腔镜辅助小切口治疗创伤性连枷胸具有伤口小,易恢复,住院时间短,并发症少等优点,疗效显著。Objective To investigate the clinical effect of thoracoscopic assisted small incision in the treatment of traumatic flail chest.Methods A total of 52 patients with traumatic flail chest who underwent thoracic surgery from April 2010 to October 2017 were convenient selected and treated with thoracoscopy-assisted small incision to treat intrathoracic injuries,including removal of hemorrhage,exploration of hemostasis,and repair of pulmonary laceration surgery,etc.,then the rib fracture was treated,and the rib fixation was performed according to the location and number of rib fractures under thoracoscopy.The operation time,hospital stay,postoperative drainage,and postoperative complications were observed.Results The average operation time of the 52 patients with thoracotomy was(70.9±4.6)min,the average hospital stay was(13.4±5.3)days,and the average drainage volume was(289.2±9.8)mL.No serious complications occurred in the entire group.Conclusion Thoracoscopy-assisted small incision for the treatment of traumatic flail chest has the advantages of small wound,easy recovery,short hospital stay and less complications.
