摘 要:目的探讨钻孔引流术和开颅血肿清除术治疗小脑出血临床疗效及安全性差异。方法研究对象方便选取该院2015年7月—2018年4月收治小脑出血患者共80例,以随机数字表法分为A组(40例)和B组(40例),分别采用开颅血肿清除术和钻孔引流术治疗,比较两组围手术期临床指标水平,治疗前后CSS评分、ADL-BI评分及术后并发症发生率。结果 A组患者手术时间和住院时间分别为(95.56±16.47)min,(15.40±3.84)d;B组患者手术时间和住院时间分别为(41.18±7.20)min,(10.12±2.57)d;B组手术时间和住院时间均显著少于A组,差异有统计学意义(t=4.17、4.35,P=0.00、0.00);两组治疗后CSS评分和ADL-BI评分比较差异无统计学意义(t=1.17、0.94,P=0.46、0.64);两组术后并发症发生率比较差异无统计学意义(χ~2=1.59,P=0.37)。结论钻孔引流术和开颅血肿清除术治疗小脑出血具有相近神经功能和生活质量改善效果,其中钻孔引流术应用有助于降低操作难度,缩短术后康复进程及避免相关并发症出现。Objective To investigate the clinical efficacy and safety of bursal drainage and craniotomy hematoma evacuation in the treatment of cerebellar hemorrhage.Methods A total of 80 patients with cerebellar hemorrhage admitted to the hospital from July 2015 to April 2018 were randomly divided into group A(40 cases)and group B(40 cases)with craniotomy.Surgery and burr drainage were used to compare the perioperative clinical indicators of the two groups,CSS score,ADL-BI score and postoperative complication rate before and after treatment.Results The operation time and hospitalization time of group A were(95.56±16.47)min,(15.40±3.84)d.The operation time and hospitalization time of group B were(41.18±7.20)min,(10.12±2.57)d;The operation time and hospitalization time were significantly lower than in group A,the different was statistically significant(t=4.17,4.35,P=0.00,0.00).There was no significant difference in CSS score and ADL-BI score between the two groups after treatment(t=1.17,0.94,P=0.46,0.64);there was no significant difference in the incidence of postoperative complications between the two groups(χ2=1.59,P=0.37).Conclusion Drilling drainage and craniotomy hematoma evacuation for cerebellar hemorrhage have similar neurological function and quality of life improvement.The application of drilling drainage can help reduce the difficulty of operation,shorten the postoperative recovery process and avoid related complications.
