摘 要:目的探析高频彩色多普勒超声在乳腺肿块诊断中的临床价值。方法方便选取2016年8月—2017年12月该院收治的60例乳腺肿块患者为主要研究对象,所有患者均经术中、术后病理确诊,且术前均接受二维超声、高频彩色多普勒超声两种影像学方法检查,对两种影像学的诊断结果与手术病理结果进行对比分析。结果高频彩色多普勒超声在乳腺肿块上的诊断准确率、特异度和敏感度均明显高于二维超声(100.00%vs85.00%,96.67%vs83.33%,95.00%vs81.67%),两组对比差异有统计学意义(χ~2=9.729、5.926、5.175,P=0.002、0.015、0.023<0.05)。恶性乳腺肿瘤患者声像图的血流显示率明显高于良性乳腺肿瘤患者,差异有统计学意义(7.14%vs38.89%,χ~2=9.143,P=0.000<0.05);恶性乳腺肿瘤患者的血流阻力指数明显高于良性乳腺肿瘤患者(0.49±0.08)vs(0.75±0.05),两组对比差异有统计学意义(t=12.728,P=0.000<0.05),但良性和恶性乳腺肿瘤患者的血流峰值对比[(13.52±2.26)cm/s vs 14.01±2.35)cm/s],差异无统计学意义(t=0.761,P=0.450>0.05)。结论高频彩色多普勒超声诊断乳腺肿块的准确性高,安全有效,值得在临床上进一步推广应用。Objective To analyze the clinical value of high frequency color Doppler ultrasound in the diagnosis of breast mass.Methods Sixty patients with breast masses admitted to the hospital from August 2016 to December 2017 were convenient selected as the main subjects.All patients were diagnosed by intraoperative and postoperative pathology,and received two-dimensional ultrasound and high-frequency color Doppler ultrasound two imaging methods were used to compare the diagnostic results of the two imaging studies with the surgical pathological results.Results The diagnostic accuracy,specificity and sensitivity of high-frequency color Doppler ultrasound on breast masses were significantly higher than those of two-dimensional ultrasound(100.00%vs 85.00%,96.67%vs 83.33%,95.0%vs 81.67%).The difference was statistically significant,(χ2=9.729,5.926,5.175,P=0.002,0.015,0.023<0.05).The blood flow rate of sonograms in patients with malignant breast tumors was significantly higher than that in patients with benign breast tumors(7.14%vs 38.89%).The difference was statistically significant(χ2=9.143,P=0.000<0.05);blood of patients with malignant breast tumors flow resistance index was significantly higher than that of benign breast tumor patients.There was statistically a significant difference between the two groups[(0.49±0.08)vs(0.75±0.05)t=12.728,P=0.000<0.05)],but the peak blood flow of patients with benign and malignant breast tumors was compared[(13.52±2.26)cm/s vs 14.01±2.35)cm/s].The difference was not statistically significant(t=0.761,P=0.450>0.05).Conclusion High-frequency color Doppler ultrasonography is highly accurate,safe and effective in the diagnosis of breast masses.It is worthy of further application in clinical practice.
