摘　　要：目的观察温针灸联合血栓通注射液治疗对急性脑梗死患者脑血流CT灌注情况、脑血流动力学的影响。方法 80例急性脑梗死患者随机分为观察组和对照组,每组40例。对照组给予常规药物治疗及康复措施,观察组在对照组基础上加用温针灸联合血栓通注射液治疗。比较两组治疗前后脑CT灌注成像指标、脑血流动力学的变化。结果两组治疗后Vp、Vm高于同组治疗前(P<0.01),DVp、DVm低于同组治疗前(P<0.01);观察组治疗后Vp、Vm高于对照组(P<0.01,P<0.05),DVp、DVm低于对照组(P<0.01,P<0.05)。两组治疗后CBF、CBV水平较同组治疗前增高(P<0.05,P<0.01),MTT降低(P<0.01);观察组治疗后CBF、CBV水平高于对照组(P<0.05),MTT水平低于对照组(P<0.05)。两组治疗后NIHSS评分较同组治疗前降低(P<0.01),观察组NIHSS评分低于对照组(P<0.01)。两组FMA评分均较同组治疗前提高(P<0.01),观察组FMA评分高于对照组(P<0.05)。结论温针灸联合血栓通注射液可显著改善急性脑梗死患者脑血流灌注水平及脑血流动力学,从而有助于神经功能康复。Objective To observe the effect of warm needling plus Xue Shuan Tong injection on cerebral perfusion based on head CT and cerebral hemodynamics in patients with acute cerebral infarction.Method Eighty patients with acute cerebral infarction were randomized into an observation group and a control group,with 40 cases in each group.The control group was intervened by conventional medication and rehabilitation,while the observation group was additionally given warm needling plus Xue Shuan Tong injection.The changes in head CT indicators of cerebral perfusion and cerebral hemodynamics were compared between the two groups.Result After the treatment,the peak velocity(Vp)and mean velocity(Vm)increased(P<0.01)and the symmetrical difference in Vp(DVp)and symmetrical difference in Vm(DVm)declined(P<0.01)in the two groups;the Vp and Vm in the observation group were higher than those in the control group after the treatment(P<0.01,P<0.05)while the DVp and DVm in the observation group were lower than those in the control group(P<0.01,P<0.05).The cerebral blood flow(CBF)and cerebral blood volume(CBV)increased(P<0.05,P<0.01)and the mean transit time(MTT)decreased(P<0.01)after the treatment in both groups;the levels of CBF and CBV in the observation group were higher than those in the control group after the treatment(P<0.05)and the MTT level was lower in the observation group than in the control group(P<0.05).After the treatment,the score of National Institute of Health Stroke Scale(NIHSS)dropped in the two groups(P<0.01)and the NIHSS score in the observation group was lower than that in the control group(P<0.01).The Fugl-Meyer Assessment(FMA)score increased after the intervention in both groups(P<0.01)and the FMA score in the observation group was higher than that in the control group(P<0.05).Conclusion Warm needling plus Xue Shuan Tong injection can significantly improve the cerebral perfusion and hemodynamics in patients with acute cerebral infarction,so that it’s beneficial to the recovery of neural function.
