摘  要:目的探讨氧化苦参碱对异丙肾上腺素(ISO)诱导心力衰竭大鼠的心脏保护作用及其可能机制。方法 42只SD大鼠,随机分为空白对照组、异丙肾上腺素模型组、氧化苦参碱对照组、氧化苦参碱治疗组(100、50、25mg/kg),每组7只,建立充血性心衰模型并分组处理。检测心功能血流动力学改变及心肌组织病理变化,并通过蛋白芯片技术检测选取的各组动物心肌组织中20种细胞因子的表达水平。结果与空白对照组比较,异丙肾上腺素模型组脑钠肽、肌钙蛋白Ⅰ含量,左心室内压最大下降速率(-LVdP/dtmin)均明显升高(均P<0.01),左心室收缩压、心率和左心室内压最大上升速率(+LVdP/dtmax)均显著降低(均P<0.01)。异丙肾上腺素模型组大鼠心肌组织中CINC-3的表达水平显著升高(P<0.05),金属蛋白酶组织抑制因子(TIMP)-1表达水平显著升高(P<0.01);与异丙肾上腺素模型组比较,各剂量氧化苦参碱(100、50、25mg/kg)治疗均可显著降低血清脑钠肽、肌钙蛋白Ⅰ含量,并降低心肌组织中共刺激分子B7-2,中性粒细胞趋化因子(CINC-2、CINC-3)的表达水平(均P<0.05)。结论氧化苦参碱对异丙肾上腺素诱导的慢性心力衰竭大鼠的心脏保护作用机制可能与降低细胞因子B7-2、CINC-2、CINC-3的表达水平密切相关。所选取的细胞因子中,可能以TIMP-1的表达变化与心力衰竭的发生机制关系最为密切。Objective To investigate the protective effect of oxymatrine on the heart in rats with congestive heart failure induced by isoproterenol(ISO)and its underline mechanism.Methods T'he SD rats(n=4 2.)were randomly divided into control group(n=7.),isoprotcrcnoi model group(n=7),oxymatrine control group(n=7.)and oxymatrine treatment groups(100,50,25 mg/kgn=7 each).The rat modri of congestive heart failure was established and the expression of 27 cytokines in myocardial tissue was analyzed by protein chip assay.Results Compared with the blank control groups the levels of brain natriuretic pep-tidc(BNP.)and troponin I(Tn I).the maximum decrease rate of left ventricular pressure(-LVdP/dtmin)and the expression levels of CINC-3 andTIMP-1 in myocardial tissue were significantly increased,and the left ventricular systolic pressure,heart rate.the maximal increase rate of lett ventricular prcssurc(+LVdP/dtnux)and the level of VEGF were significantly decreased in the isoproterenol model group(P〈0.05 or P〈0.01 for all.).Compared with the isoprotcrcnolmodcl group,oxymatrinc(100,50,5 mg/kg.)significantly reduced BNP,Tne,and cstimulatory molecule B7-2,nd neutrophil chcmotactic factorsCCINC-2,CINC-3).Conclusion Oxymatrine can protect against ISO-induced congestive heart failure in rats?which may be correlated with the decreases of cytokines B7-2.CINC-2 andCINC-3.Among the cytokines examined,the expression level ofTIMP-1 may be most closely related to the mechanism of heart failun.
