光子治疗仪联合烤灯照射在头颈部皮瓣修复术后的应用

摘　　要：目的观察在光子治疗仪联合烤灯照射在头颈部皮瓣修复术后中的应用效果观察。方法选择我院2016年8月～2018年6月头颈部面部软组织缺损患者90例,按照随机数字表法分为2组各45例。所有患者均给予常规护理。对照组自术后1天起行烤灯照射治疗,共治疗14天。观察组自术后1天行烤灯照射治疗,第5d开始行光子治疗仪治疗,至术后14天停止。记录和比较两组患者的皮瓣肿胀消失时间、创面液体渗出消失时间,伤口愈合时间,术后第1d、术后第7d的皮瓣颜色,术后14天后PSQI得分。记录和比较两组患者术后14h的并发症。结果观察组创面液体渗出消失时间、皮瓣肿胀消失时间与对照组差异无统计学意义(P> 0.05)。所有患者皮瓣均为I期愈合。观察组皮瓣切口愈合时间为短于对照组(P <0.05)。术后7d两组皮瓣颜色比较差异并无统计学意义(P> 0.05)。术后14d,观察组PSQI各项及总分均低于对照组(P <0.05)。观察组术后感染2例与对照组术后感染3例比较差异并无统计学意义(P> 0.05)。结论光子治疗仪联合烤灯照射在头颈部皮瓣修复术后可发挥促进伤口愈合,提高患者的生活质量。Objective To observe the effect of phototherapy combined with flue-cured lamp irradiation in the repair of head and neck flap.Methods 90 patients with head and neck soft tissue defects from August 2016 to June 2018 were selected and divided into two groups according to the random number table.All patients were given routine care.The control group received the treatment of 1 days after the operation.The treatment lasted for 14 days.The observation group received lamp irradiation one day after operation,photon therapy apparatus was used on the 5th day,and stopped 14 days after operation.The time of flaps swelling disappearance,the time of wound fluid exudation disappearance,the time of wound healing,the color of flaps on the first day and the seventh day after operation were recorded and compared.PSQI scores were scored 14 days after operation.Postoperative complications of 14h in two groups were recorded and compared.Results There was no significant difference between the observation group and the control group in the disappearance time of wound fluid exudation and skin flap swelling(P>0.05).All the flaps healed by I.The healing time of flap in observation group was shorter than that in control group(P<0.05).There was no significant difference in color of flap between 7d and two groups after operation(P>0.05).After 14d,the PSQI and the total score of the observation group were all lower than those of the control group(P<0.05).There was no significant difference in postoperative infection between 2 cases in observation group and 3 cases in control group(P>0.05).Conclusion Photon therapy apparatus combined with flue-cured lamp irradiation can promote wound healing and improve the quality of life of patients after head and neck flap repair.
