滋肾健脾化瘀方联合主动免疫治疗对复发性流产肾虚血瘀证患者的临床疗效

摘　　要：目的探讨滋肾健脾化瘀方联合主动免疫治疗对复发性流产肾虚血瘀证患者的临床疗效。方法 240例患者随机分为滋肾健脾化瘀方组、主动免疫治疗组、联合组,每组80例,测定中医证候评分、足月分娩率、不良反应发生率,并检测血清炎症因子[肿瘤坏死因子-α(TNF-α)、D-二聚体(DIM)、白细胞介素-4 (IL-4)、白细胞介素-12(IL-12)]、封闭抗体[CD3、CD4、CD8、CD4/CD8]水平。结果与其他2组比较,联合组足月分娩率及孕期12周IL-4、CD8水平显著增加(P <0. 05),孕期12周中医证候总评分及TNF-α、DIM、IL-12、CD3、CD4、CD4/CD8水平显著降低(P <0. 05)。3组不良反应发生率依次为滋肾健脾化瘀方组<联合组<主动免疫治疗组,差异有统计学意义(P <0. 05)。结论滋肾健脾化瘀方联合主动免疫治疗对复发性流产肾虚血瘀证患者具有增效减毒的作用。AIM To explore the clinical effects of Zislien Jianpi Huayu Decoction combined witli active immu-notlierapy on recurrent spontaneous abortion patients due to Kidney Deficiency and Blood Stasis Pattern.METHODS Two hundred and forty patients were randomly divided into Zislien Jianpi Huayu Decoction group,active immunotherapy group and combination group,80 cases in each group.TCM syndrame score,full-term deliv-er rte and adverse reaction occurrence rate were determined,and the levels of serum inflammator factors[tumor necrosis factor-{&(TNF-a),D-dimer(DIM),intrleukin-4(IL-4),intrleukin-12(IL-12)],blocking antibod-ies(CD3,CD4,CD8,CD4/CD8)were detected.RESULTS Compared withi the othier two groups,the combi-nation group demonstrated significantly increased full-term delivery rate and IL-4,CD8 kvvls at the 12th week of pregnancy(P<0.05),markedly decreased total TCM syndrome score and TNF-a,DIM,IL-12,CD3,CD4,CD4/CD8 levels at tiie 12th week of pregnancy(P<0.05).Adverse reaction occurrence rates in the three groups were in sequence of theZishen Jianpi Huayu Decoction group<thecombination group<theactive immunotherapygroup,thedifferences had statistical significances(P<0.05).CONCLUSION For recuirent spontaneous abor-tion patients due to Kidney Deficiency and Blood Stasis Pattern,Zisiien Jianpi Huayu Decoction combined witii ac-tive immunotiierapy exhibits efficacy enhancing and toxicity reducing effect.
