攀枝花地区H型高血压血浆同型半胱氨酸水平与脑卒中的相关性分析

摘　　要：目的探讨攀枝花地区H型高血压血浆同型半胱氨酸水平与脑卒中的关系。方法选择2015年9月至2016年7月在我院进行H型高血压调查患者2290例,根据是否发生脑卒中患者被分为非脑卒中组1963例(85.72%)与脑卒中组327例(14.28%),根据血浆Hcy水平分为A组(1890例,<20μmol/L)、B组(278例,20～30μmol/L)和C组(122例,>30μmol/L),分析患者临床基线资料与血浆Hcy水平。结果脑卒中组血浆Hcy水平(21.02±15.47μmol/L)高于非脑卒中组患者(18.01±9.34μmol/L),差异有统计学意义(P<0.05);在脑卒中发病率,C组(31.15%)与B组(23.02%)或A组(11.90%)之间比较,B组与A组之间比较,差异均有统计学意义(P<0.01);线性回归分析显示血浆Hcy水平与脑卒中发病率呈正相关性(r=0.163,P<0.01)。结论 H型高血压患者应积极降压和降低血浆Hcy水平是预防脑卒中发生的关键环节。Objective To investigate the relationship between plasma homocysteine level and stroke in H-type hypertension in Panzhihua area.Methods 2290 cases of H-type hypertensionin our hospital from September 2015 to July 2016 were investigated.According to whether had stroke,patients were divided into non-stroke group,1963 cases(85.72%)and stroke group,327 cases(14.28%).According to plasma homocysteine level,patients were divided into group A(1890 cases,<20μmol/L),group B(278 cases,20-30μmol/L)and group C(122 cases,>30μmol/L)and analyze patients clinical baseline data and plasma homocysteine levels.Results The plasma homocysteine level in strokegroup(21.02±15.47μmol/L)was higher than that in non-stroke group(18.01±9.34μmol/L),the difference was statistically significant(P<0.05).In stroke incidence,group C(31.15%)compared group B(23.02%)or group A(11.90%),group B compared group A,the difference was statistically significant(P<0.01).Linear regression analysis showed that the plasma homocysteine level was positively correlated with the incidence of stroke(spearma n=0.163,P<0.01).Conclusion H-type hypertension patients should actively reduce blood pressure and plasma homocysteine level,which are the key steps to prevent the occurrence of stroke.
