急诊手术与择期手术在Gartland Ⅲ型儿童肱骨髁上骨折中的疗效比较

摘　　要：目的 Gartland Ⅲ型儿童肱骨髁上骨折行急诊手术及择期手术的疗效对比。方法对2015年5月至2016年6月经我院急诊科收治筛选后的68例Gartland Ⅲ型儿童肱骨髁上骨折患儿进行回顾性研究。将其分为急诊手术A组男18例,女16例,平均年龄(6.74±0.77)岁及择期手术B组男19例,女15例,平均年龄(6.55±0.85)岁,两组均拟行闭合复位交叉克氏针固定方式手术。两组患儿均获得1年时间随访,术后对比两组患儿手术时间,术中透视次数,术中行切开复位比例,术后1年患儿Flynn肘关节功能评分。结果急诊手术A组手术时间较择期手术B组明显缩短(25.19±2.54)min vs(37.22±3.14) min;术中透视次数A组较B组明显减少(12.13±1.32)次vs(17.52±2.06)次;术中B组行切开复位内固定术比例较A组明显升高(10%vs 2%);术后患儿Flynn肘关节功能评分未见明显统计学差异。结论急诊手术可缩短手术时间减少术中透视次数及转行切开复位内固定术比例。Objective To compare the effect of emergency operation and selective operation in children with supracondylar fracture of humerus of Gartland typeⅢ.Methods A retrospective study of 68 children with supracondylar fracture of humerus of Gartland typeⅢtreated in the emergency department of our hospital from May 2015 to June 2016 was conducted.They were divided into emergency operation group A,18 males and 16 females,with an average age of(6.74±0.77)and selective operation group B,19 males and 15 females,with an average age of(6.55±0.85).Both groups were scheduled to undergo closed reduction and cross Kirschner wire fixation.All the children in both groups were followed up for one year.After operation,the operation time,the times of fluoroscopy,the proportion of open reduction and the Flynn elbow function score 1 year after operation were compared between the two groups.Results The operation time of emergency operation in group A was significantly shorter than that of selective operation in group B(25.19±2.54)min vs(37.22±3.14)min;The frequency of intraoperative fluoroscopy in group A was significantly lower than that in group B(12.13±1.32)times vs(17.52±2.06)times;The proportion of open reduction and internal fixation in group B was significantly higher than that in group A during operation 10%vs2%;There was no significant difference in Flynn elbow function score between the two groups after operation.Conclusion Emergency operation can shorten the operation time,reduce intraoperative fluoroscopy times and the proportion of open reduction and internal fixation.
