恩度联合洛铂胸腔热疗治疗恶性胸腔积液的临床观察

摘　　要：目的观察重组人血管内皮抑制素注射液(恩度)联合洛铂、胸腔热疗治疗恶性胸腔积液的有效性和安全性。方法将确诊为恶性胸腔积液的60例患者随机分成两组,每组30例,对照组胸腔内注射洛铂30 mg+恩度45 mg,观察组胸腔内注射洛铂30 mg+恩度45 mg,注药后1 h进入热疗室治疗。比较两组间疗效、生活质量和毒副作用。结果观察组控制胸水总有效率高于对照组(P<0.05)。观察组治疗后生活质量改善率明显高于对照组(P<0.05);在毒副作用方面两组无明显差异。结论洛铂联合恩度及热疗治疗恶性胸腔积液近期疗效好,改善患者的临床症状,提高其生活质量,同时延长药物产生耐药时间。Objective:To observe the clinical effect and safety of recombinant human endostatin(Endostar)and lobaplatin combined with thoracic local thermotherapy for the treatment of malignant pleural effusion.Methods For a total of 60 patients with malignant pleural effusion were randomly divided into observa-tiongroup and control group,30 cases in each group.All patient were treated with thoracentesis and inserted closed thoracic drainage tube tu drainage of pleural effusion.After exhaustion of pleural effusion,the control group was treated with endostar(45 mg)and lobaplatin(30 mg)by perfusion,the trial group was treated with endostar(45 mg)and lobaplatin(30 mg)by perfusion,then the trial group were given local thermotherapy 60 minutes after thoracic 1 perfusion.The efficacy,quality of life and side effects between the 2 groups were compared.Results The total effective rate of pleural effusion in the observation group was 86.67%,higher than 66.67%in the control group,and the difference was statistically significantCP^0.05).The improvement rate of quality of life in the observation group was 90%,higher than that in the control group 66.67%(P<0.05).There was no significant difference in the toxic and side effects between the 2 groups.There was no significant difference between the two groups in the observation of the improvement of the quality of life in the observation group.Conclusion lobaplatin combined with Endostar and hyperthermia in the treatment of malignant pleural effusion,good curative effect,improve clinical symptoms of patients,so as to improve the quality of life.
