红景天联合复方麝香注射液对小儿先天性心脏病术后相关指标的临床研究

摘　　要：目的探讨红景天联合复方麝香注射液对于小儿先天性心脏病术后相关指标的影响,评估两药联合对小儿先心病术后恢复的临床疗效及药物联合应用的安全性。方法选取200例择期行心肺转流下小儿先心病矫治术(CPB)的患者,按照尊重治疗意愿的方式,采用平均分组的方法,将200例患者分为对照组和研究组,每组100例,对照组给予常规生活方式干预及临床对症治疗,研究组在对照组的基础上给予红景天注射液(1 mg/kg)于术前1 d、术中以及术后7 d连续静脉滴注联合复方麝香注射液(1. 0 ml/kg)于术中停用麻药后静脉泵入30 min,并分别于术前1 d、术后第1天、术后第5天分别观察2组患者的肿瘤坏死因子-α(TNF-α)、白介素-2(IL-2)、白介素-10(IL-10)、泌乳素、雌二醇的变化情况,治疗结束后对2组患者的临床疗效及安全性进行分析总结。结果治疗前2组患者的性别比、年龄、体重及相关实验室指标比较差异无统计学意义(P> 0. 05)。治疗后2组患者的TNF-α、IL-2、IL-10、泌乳素以及雌二醇指标恢复正常水平,差异均有统计学意义(P <0. 05);研究组CPB术后上述指标浓度明显高于对照组,差异有统计学意义(P <0. 01); 2组术后疗效性比较上治疗后优于治疗前,且研究组明显优于对照组(P <0. 05); 2组术后安全性比较差异无统计学意义(P> 0. 05)。结论红景天联合复方麝香注射液对炎性因子、抗炎性因子、泌乳素、雌二醇以及小儿的早期认知功能障碍有较好的调节作用,对防治CPB后炎性反应、认知性障碍及维持机体性激素的正常水平有极高的临床应用价值,且其安全系数高社会的和谐稳步发展意义重大,值得临床进一步推广应用。Objective To investigate the effects of Hongjingtian injection combined with compound musk injection on related indexes of children with congenital heart disease(CHD)after surgery,and to evaluate the postoperative recovery effects and safety of combination application of the two drugs.Methods A total of children patients with CHD who underwent cardiopulmonary bypass CHD surgery(CPB)in our hospital from October 2015 to October 2017 were randomly divided into control group(n=100)and research group(n=100).The patients in control group were given conventional lifestyle intervention and clinical symptomatic treatment,however,the patients in research group,on the basis of control group,were given Hongjingtian injection(1mg/kg)combined with compound musk injection at 1 day before surgery,during surgery and postoperative 7 days by continuous intravenous drip(1.0ml/kg).The changes of serum levels of tumor necrosis factor-α(TNF-α),interleukin 2(IL-2),interleukin 10(IL-10),prolactin,estradiol,and the clinical curative effects and safety were observed and compared between the two groups.Results Before treatment there were no significant differences in the patient’s sex and age,body weight and related laboratory indexes between the two groups(P>0.05).After treatment there were significant differences in the levels of TNF-α,IL-2,IL-10,prolactin and estradiol between the two groups(P<0.05),moreover above indexes in research group after CPB were significantly higher than those in control group(P<0.01).The therapeutic effects after CPB in both groups were superior to those before CPB,moreover,which in research group superior to those in control group(P<0.05).There was no significant difference in posyoperative safety between the two groups(P>0.05).Conclusion Hongjingtian injection combined with compound musk injection has better regulatory effects on inflammatory factors,anti-inflammatory factors,estradiol and prolactin as well as early cognitive dysfunction of children with CHD after CPB,which can prevent and treat the infl
