摘　　要：目的探讨骨折术后卧床患者出院前常规行彩色多普勒超声(color doppler flow imaging,CDFI)检查下肢深静脉血栓(DVT)的诊断价值及临床意义。方法骨折术后卧床患者200例出院前常规进行CDFI检查,观察DVT发生情况。结果 200例患者中,CDFI共检查出88例DVT形成,发生率44%,其中腘静脉血栓5例,股浅静脉血栓8例,胫后静脉血栓12例,腓静脉血栓21例,小腿肌间静脉血栓42例。24例患者在术前及术后检查CDFI时均未发现血栓,但出院前B超示血栓形成,发生率为17. 65%。其中腘静脉血栓1例,胫后静脉血栓2例,股浅静脉血栓4例,腓静脉血栓4例,肌间静脉血栓13例。结论骨折卧床患者出院前常规进行CDFI检查对于下肢深静脉血栓的早期诊断和早期治疗具有重要意义,对于降低骨折术后肺栓塞的风险具有很高的价值,值得临床推广。Objective To investigate the diagnostic value and clinical significance of conventional examination of color Doppler flow imaging(CDFI)for deep venous thrombosis(DVT)of patients with bone fracture after surgery and before being discharged from hospital.Methods The clinical data about 200 patients with bone fracture who underwent CDFI examination in our hospital before being discharged from hospital from November 2017 to May 2018 were retrospectively analyzed,and the incidence of lower extremity DVT(DVT)was observed.Results Among the 200 patients,there were 88 cases of DVT detected by CDFI,with the incidence rate of DVT being 44%,in whom,there were 5 cases of popliteal vein thrombosis,8 cases of superficial femoral vein thrombosis,12 cases of posterior tibial vein thrombosis,21 cases of fibula thrombosis and 42 cases of intermuscular venous thrombosis.No thrombus was detected in 24 patients before and after surgery by CDFI examination,but the incidence of thrombosis was 17.65%before being discharged from hospital,in whom,there was 1 case of popliteal vein thrombosis,2 cases of posterior tibial vein thrombosis,4 cases of superficial femoral vein thrombosis,4 cases of fibular vein thrombosis and 13 cases of intermuscular venous thrombosis.Conclusion The routine CDFI examination is of great significance for the early diagnosis and early treatment of deep venous thrombosis of the lower extremities.It is of high value for reducing the risk of pulmonary embolism after fracture,therefore,it is worthy of clinical popularization.
