腱鞘巨细胞瘤的MRI表现及不同病理类型对照分析

摘　　要：目的探讨腱鞘巨细胞瘤(Tenosynovial giant cell tumor,TGCT)的MRI特征,并比较不同病理类型TGCT的MRI表现差异。方法回顾性分析18例经手术病理证实为TGCT患者的MRI影像学资料,观察病变位置、病灶大小、边界、平扫及增强MRI信号特点、累及范围、是否出现关节积液及滑膜增生等,比较不同病理类型TGCT相关MRI征象的差异。结果 18例TGCT包括局限型10例、弥漫型8例,二者在病灶大小、边界、MRI平扫信号均匀性、对周围骨质的侵袭等方面差异有统计学意义(P <0. 05)。结论 TGCT的MRI表现具有一定特征性,不同病理类型TGCT的MRI表现存在一定差异性,仔细分析相关MRI征象可对TGCT做出较为正确的术前诊断,并能进一步区分其病理学类型,指导临床选择恰当的治疗方案。To explore the MRI imaging findings of tenosynovial giant cell tumor(TGCT)and compare the differences of MRI signs among different pathological types of TGCT.MRI manifestations of 18 patients with TGCT confirmed by surgery and pathology were retrospectively analyzed.The location,size and boundary of each lesion,characteristics of plain and enhanced scan,involvement of surrounding soft tissue and appearance of joint effusion and synovial hyperplasia were observed.The differences of MRI signs among different pathological types of TGCT were compared.The 18 TGCT patients included 10 localized and 8 diffused types.There was a statistical difference between the two types of TGCT in size,boundary,homogeneity and intensity of plain scan signals,the involvement of surrounding soft tissue and bone invasion(P<0.05).The MRI manifestations of TGCT have certain characteristics,and there are some differences in the different two pathological types.Careful analysis of the MRI signs of TGCT can make a relatively correct preoperative diagnosis and further differentiate the pathological types of TGCT.Therefore,it could provide imaging support for the clinicians to adopt right treatments.
