摘  要:目的探讨使用肝素锂抗凝采血管对电化学发光法检测甲状腺激素及相关抗体的影响。方法随机选取60例华西医院门诊患者,同时采用美国BD公司无添加剂的红头管和肝素锂抗凝的绿头管收集患者血液标本,离心后用罗氏Cobas e601型电化学发光免疫分析仪测定甲状腺激素及其相关抗体并比较两种样本间检测结果的差异,检测项目包括:游离三碘甲酰原氨酸(FT_3)、游离甲状腺素(FT_4)、促甲状腺激素(TSH)、甲状腺球蛋白(TG)、三碘甲酰原氨酸(T_3)、甲状腺素(T_4)、促甲状腺激素受体抗体(TRAb)、抗甲状腺球蛋白抗体(A-TG)和抗过氧化物酶抗体(A-TPO)。结果血清样本和肝素锂抗凝血浆样本中FT_3、FT_4、T_3、T_4和TSH检测结果差异无统计学意义(P> 0. 05),而肝素锂抗凝血浆样本中A-TG和TRAb检测浓度明显高于血清样本,差异有统计学意义(P <0. 05)。结论肝素锂可干扰A-TG和TRAb测定,检测该两种抗体应使用血清样本。To investigate the effect of heparin lithium anticoagulant on the detection of thyroid hormones and related antibodies by electrochemiluminescence.Sixty outpatients in our hospital were randomly selected.Blood samples were simultaneously collected by red vacuum tube and green tube with heparin lithium for the determination of thyroid hormones and thyroid related antibodies including free triiodothyronyl acid(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH),thyroglobulin(TG),triiodoyl(T3),thyroxine(T4),thyrotropin receptor antibody(TRAb),anti-thyroglobulin antibody(A-TG)and anti-peroxidase antibody(A-TPO).All the tests were completed on Roche Cobas e601 electrochemiluminescence immunoassay analyzer.The results of green tube and red tube were compared.There was no significant difference in assay results of serum FT3,FT4,T3,T4,TSH,TG and A-TPO between the red tubes and the green tubes(P>0.05),while plasma concentrations of A-TG and TRAb in the green tubes were significantly higher than those in the red tubes(P<0.05).Heparin lithium can affect the determination of A-TG and TRAb.It is recommended to use serum samples to detect these two antibodies.
