医院门诊处方审核情况分析与探讨

摘　　要：目的分析医院门诊处方审核干预实例,查找不合理处方产生原因,促进临床合理用药,保障患者用药安全。方法依据药品说明书、《处方管理办法》、《医院处方点评管理规范(试行)》、《抗菌药物临床应用指导原则》等相关法律法规及规章制度,对2016年7～12月门诊药师审核干预的处方进行回顾性分析。结果 2016年7月～12月门诊药师共调剂处方1668438张,其中共干预处方21069张,占总处方数的1. 26%。在所干预的处方中,不规范处方8191张,占38. 88%,用药不适宜处方11221张,占53. 26%,超常处方1657张,占7. 86%。结论药师应加强处方用药适宜性审核,不断完善处方干预标准,提高处方质量和合理用药水平,保障患者用药安全。To analyze the hospital outpatient prescription reviews and find the cause of unreasonable prescriptions in order to promote the rational use of medicine and ensure the safety of patients.The prescription reviews in our hospital from July 2016 to December 2016 were retrospectively analyzed according to drug manual,“Prescription management approach”,“Hospital prescription review s management practices(Trial)”and“Antimicrobial clinical application guidelines”.There were 1668438 prescriptions were reviewed by clinical pharmacists from July 2016 to December 2016,in which 21069 prescriptions were intervened,accounting for 1.26%.Among these intervened prescriptions,8191(38.88%)were non-standard prescriptions,11211(53.26%)were inappropriate prescriptions,and 1657(7.86%)were prescriptions beyond indications.Clinical pharmacists should strengthen to review the appropriateness of prescribed drugs,prevent the inappropriate prescriptions before hand,and reduce the potential medical risk in time.Thus,we could improve the quality of the prescriptions and the level of appropriate drug usage and ensure the safety of administrated patients.
