摘 要:目的探讨标本溶血对生化检验结果准确性的影响及相关预防对策。方法 170例健康体检人员,依照临床标准手段进行生化检验,将被检者空腹状态下10 ml静脉血标本随机分为2份,每份5 ml。两份血液标本分别在试管中进行常规处理和溶血处理,比较常规与溶血处理后白蛋白、总蛋白、葡萄糖、血清钾、乳酸脱氢酶、谷丙转氨酶、谷草转氨酶、总胆红素、尿素氮及肌酸激酶水平。结果常规处理后总蛋白、葡萄糖、血清钾、乳酸脱氢酶、谷草转氨酶、总胆红素、肌酸激酶水平分别为(59.11±6.65)g/L、(4.65±0.73)mmol/L、(3.66±0.44)mmol/L、(20.04±30.16)U/L、(19.04±11.37)U/L、(10.22±5.03)μmol/L、(73.46±22.07)U/L,溶血处理后总蛋白、葡萄糖、血清钾、乳酸脱氢酶、谷草转氨酶、总胆红素、肌酸激酶水平分别为(80.17±6.67)g/L、(2.61±1.15)mmol/L、(5.59±0.86)mmol/L、(45.86±53.94)U/L、(33.24±10.59)U/L、(29.84±5.68)μmol/L、(105.33±20.67)U/L;两种检验方法中总蛋白、葡萄糖、血清钾、乳酸脱氢酶、谷草转氨酶、总胆红素、肌酸激酶水平比较差异有统计学意义(P<0.05);两种检验方法中白蛋白、尿素氮及谷丙转氨酶水平比较,差异无统计学意义(P>0.05)。结论溶血现象对生化检测指标产生很大影响,故此医护人员在采集、运送与保管血液样本期间,应施以有效措施规避溶血现象。Objective To discuss the influence of specimen hemolysis on accuracy of biochemical test results and preventive measures.Methods A total of 170 healthy physical examination personnel were biochemically tested according to clinical standard means.The 10 ml venous blood samples of the subjects under fasting condition were randomly divided into 2 parts,with 5 ml in each part.Two blood samples were routinely processed and hemolyzed in a test tube.The levels of albumin,total protein,glucose,serum potassium,lactate dehydrogenase,alanine aminotransferase,glutamic oxaloacetic aminotransferase,total bilirubin,urea nitrogen and creatine kinase were compared between routine and hemolysis process.Results The levels of total protein,glucose,serum potassium,lactate dehydrogenase,glutamic oxaloacetic transaminase,total bilirubin and creatine kinase were respectively as(59.11±6.65)g/L,(4.65±0.73)mmol/L,(3.66±0.44)mmol/L,(20.04±30.16)U/L,(19.04±11.37)U/L,(10.22±5.03)μmol/L and(73.46±22.07)U/L after routine process,which were(80.17±6.67)g/L,(2.61±1.15)mmol/L,(5.59±0.86)mmol/L,(45.86±53.94)U/L,(33.24±10.59)U/L,(29.84±5.68)μmol/L and(105.33±20.67)U/L after hemolysis process.There was statistically significant difference in total protein,glucose,serum potassium,lactate dehydrogenase,glutamic oxaloacetic transaminase,total bilirubin and creatine kinase between two test methods(P<0.05).There was no statistically significant difference in albumin,urea nitrogen and glutamic pyruvic transaminase levels(P>0.05).Conclusion Hemolysis has a great impact on biochemical indicators,so medical staff should take effective measures to avoid hemolysis during the collection,transportation and storage of blood samples.
