额颞叶颅脑损伤患者智能损害及执行功能的相关研究

摘　　要：目的探讨额颞叶颅脑损伤与智能及执行功能损害的关系,为法医精神病精神伤残评定提供科学依据。方法选取2015年1月至2016年8月在合肥市第四人民医院司法鉴定科进行精神、智能障碍鉴定的被鉴定人共70例,为各类事故(主要为交通事故)所致颅脑外伤患者;年龄16～65周岁,平均年龄为40.66±12.8岁,其中男46例,女24例;左侧31例、右侧39例;额叶32例、颞叶30例,其余8例为额颞叶联合伤,未列入额叶和颞叶的统计分析。应用SPSS 16.0软件对调查数据进行统计分析,比较不同组间韦氏成人智力量表及威斯康星卡片分类测验的差异。结果额颞叶颅脑损伤患者智力及执行功能普遍受损,其中左侧智力及执行功能受损大于右侧,在操作智商及执行功能上差异显著;颞叶损伤对智力及执行功能影响大于额叶,并在言语智商、总智商、部分执行功能上差异达到显著水平。结论额颞叶颅脑损伤患者智力及执行功能的损害与损伤部位密切相关。Objective To explore the correlation between the impairment of intellectual and executive functions with brain injuries in frontal lobe and temporal lobe,and to provide the scientific basis for the mental disability assessment of forensic psychiatry.Method 70 patients in the Judicial Expertise Department of the Fourth People’s Hospital of Hefei from January 2015 to August 2016,were selected.Their brain injuries were caused by various accidents(mostly traffic accident).Their ages were from 16 to 65 years old,with the average of 40.66±12.8.There were 46 males and 24 females.Among them,31 patients had left brain injury and 39 had right brain injury.Moreover,32 cases ware injured on the frontal lobe and 30 cases on the temporal lobe.The other 8 have frontal and temporal lobe joint injuries,which were not included in the statistical analysis.Results The patients with frontal lobe or temporal lobe injury all have impaired intelligence and executive functions.The impairment on the left side was greater than the right side,and the impairment in the temporal lobe was greater than the frontal lobe.Conclusion The site of brain injury in frontal lobe and temporal lobe is closely related to the impairment of intellectual and executive functions.
