摘　　要：目的对开放骨折清创术后患者住院费用及其影响因素进行分析,为开放骨折清创术后手术部位感染(surgical site infections,SSI)的预防及治疗提供依据。方法对2011至2013年住院患者电子病历数据库468例患者住院费用进行描述性统计,通过多元线性回归分析其影响因素。结果数据研究纳入468例患者,感染组244例,非感染组224例。西药费用、检查费用、护理费用、诊疗费用、手术费用、总费用的增加与SSI的发生有关(P <0. 01)。多元线性回归显示,首次清创时间、清创次数、手术次数、实际住院天数对开放骨折清创术后患者住院费用有显著性影响(P <0. 01)。结论开放骨折清创术后SSI患者住院总费用明显高于非感染患者,有效预防SSI的发生可能会节省医疗费用。Objective To analyze the hospitalization costs and influencing factors in 468 patients with open fractures after debridement in order to provide basis for the prevention and treatment of surgical site infections(SSIs)after open fractures debridement.Methods The electronic medical record data about the hospitalization costs in 468 patients with open fractures after debridement between 2011 and 2013 were descriptively statistcally analyzed,moreover,the influencing factors were analyzed by means of multiple linear regressions.Results In the 468 patients including 244 infected patients and 244 non-infected patients,the increase of western medicine costs,medical investigation costs,nursing care costs,medical service costs,surgery costs and total expenses was closely correlated with incidence of SSI(P<0.01).The multiple linear regression analyses showed that the factors affecting hospitalization costs were the first debridement time,the frequency debridement,the number of operation and the days of hospitalization(P<0.01).Conclusion The total hospitalization costs in open fracture patients with SSI after debridement are significantly higher than those of the patients without infection,to prevent effectively the incidence of SSI can save medical care costs.
