摘  要:目的探讨DR较CR对诊断肋骨骨折准确性、敏感性及其应用价。方法随机选取骨科收治的肋骨骨折患者120例,对其骨折部位进行CR、DR影像检查,并与后期临床确诊结果进行对比分析,比较两种检查方法成像准确性及对隐匿性骨折检出敏感性。结果 DR检查影像资料质量明显高于CR检查,DR成像胸片达到优级片数量明显高于CR胸片,差异有统计学意义(P <0. 01),两种方法总合格率差异无统计学意义(P> 0. 05); 120例患者经CR检查后发现219处骨折部位,DR检查后发现279处骨折部位,在采取相应的临床治疗后,确诊骨折部位共计298处,其中CR检查标记正确的181处,标记错误8处,漏诊87处; DR检查标记正确的275处,标记错误4处,漏诊23处,DR检查诊断假阳性率稍低于CR检查,但两种方法间比较差异无统计学意义(P> 0. 05),DR检查诊断肋骨骨折漏诊率明显低于CR检查,差异有统计学意义(P <0. 01);对影像易重叠的隐匿部位的成像效果比较发现,CR检查检出隐匿性骨折101处,其中正确79处,DR检查检出隐匿性骨折153处,其中正确143处,DR检查隐匿性肋骨骨折的检出率和正确率明显高于CR检查,差异均有统计学意义(P <0. 01)。结论 DR检查影像资料质量较CR检查更高,在肋骨骨折诊断的准确率及敏感性方面优于CR检查,值得在临床上推广应用。Objective To investigate and compare the accuracy and sensitivity between computed radiography(CR)and digital radiography(DR)in diagnosis of rib fracture.Methods A total of 120 patients with rib fracture who were admitted and treated in our hospital from June 2015 to June 2017 were enrolled in the study.The fracture situation was examined by computed radiography and digital radiography,respectively,and the examination results were compared with those by X ray to compare the accuracy and sensitivity between the two methods.Results The image quality of digital radiography was significantly higher than that by computed radiography,and the number of optimal class imaging films by DR was sueprior to that by CR(P<0.01).There was no significant difference in total qualification rate between the two methods(P>0.05).Among the 120 patients,219 sites of fractures were detected by CR,and 279 sites of fracture were found by DR.After corresponding clinical therapy,a total of 298 sites of fracture were diagnosed,in which,181 sites of fracture were correct by CR,and 8 sites of fracture marked by mistake and 87 sites of fracture were misdiagnosed.In comparison to that,the 275 sites of fracture were correct by DR,4 sites of fracture marked by mistake and 23 sites of fracture were misdiagnosed.The false positive rate in diagnosis of rib fracture by DR was lower than that by CR,but there was no significant differencebetween two detection methods(P>0.05).However the omission diagnose rate in diagnosis of rib fracture by DR was significantly lower than that by CR.Moreover 198 sites of unobservable fracture were diagnosed,in which,101 sites of unobservable fractures were found by CR and 79 sites were correct,and 153 sites of fracture were found by DR and 143 sites of fracture were correct,the detection rate and accuracy by DR were significantly higher than those by CR(P<0.01).Conclusion The quality of image data by DR is much better than that by CR,and the accuracy and sensitivity of DR are sueprior to those by CR,therefor,DR detection
