摘　　要：目的探讨医患治疗联盟对精神分裂症患者精神症状与日常生活能力的影响。方法选取出院临床痊愈或临床显好的精神分裂症患者104例,按随机数字表法分为对照组与试验组,每组52例。对照组患者给予常规诊疗模式治疗,试验组患者在常规治疗的基础上给予医患联盟诊疗模式治疗,2组均定期随访6个月。比较治疗前后2组患者简明精神症状(BPRS)评分及日常生活能力(ADL)评分。结果与治疗前比较,治疗后6个月2组患者缺乏活力、思维障碍及试验组患者焦虑抑郁、激活性及敌对猜疑BPRS评分显著降低,且试验组显著低于对照组(P <0. 01);与治疗前比较,治疗后1～6个月试验组ADL评分呈逐渐降低趋势(P <0. 05),而对照组ADL评分呈逐渐升高趋势(P <0. 01),且治疗后1、3、6个月试验组ADL评分显著低于对照组(P <0. 01)。结论医患治疗联盟模式可显著改善精神分裂症患者精神症状,有效提高患者的日常生活能力,且疗效显著。Objective To investigate the effects of doctor-patient alliance on mental symptom and activity of daily living of patients with schizophrenia in convalescence.Methods A total of 104 patients with schizophrenia in convalescence,who were clinically cured or recovered,were enrolled in the study.According to random digital table,these patients were divided into control group(n=52)and experiment group(n=52).The patients in control group were treated by routine diagnosis and treatment mode,however,the patients in experiment group,on the basis of control group,were treated by doctor-patient alliance diagnosis and treatment mode,with regular follow up for 6 months for both groups.The brief psychiatric rating scale(BPRS)scores and activities of daily living(ADL)scores before and after treatment were observed and compared between the two groups.Results As compared with those before treatment,the lack of energy,thought disturbance,anxiety and depression,activation and hostile suspicion,BPRS scores after 6-month treatment were significantly decreased in both groups,moreover,which in experiment group were significantly lower than those in control group(P<0.01).The ADL scores in experiment group after 1~6 month treatment had a gradual decrease tendency,however,the ADL scores in control group had a gradual increase tendency(P<0.01).In addition the ADL scores at 1m,3m,6m after treatment in experiment group were significantly lower than those in control group(P<0.01).Conclusion The doctor-patient alliance diagnosis and treatment mode can obviously improve the mental symptom of patients with schizophrenia,and can effectively improve the activity of daily living of patients,with effective therapeutic effects.
