超声诊断糖尿病肾病的研究进展

摘　　要：糖尿病肾病是慢性肾病引起微血管病变的特异性并发症,是导致终末期肾衰竭的常见原因。糖尿病肾病并发症的机制极其复杂,目前尚未完全阐明,对于其治疗,目前也尚无特异性治疗方法。糖尿病可带来一系列靶器官如心、肾、眼底的损伤,而40%的糖尿病患者会发展成为慢性肾脏病患者,进而可发展为终末期肾脏疾病。故早期诊断、早期发现糖尿病肾病不容忽视。通过研究近年的超声诊断糖尿病肾病文献,本文列举了数种糖尿病肾病的超声诊断方法,包括超声背向散射积分、彩色多普勒血流成像、彩色多普勒能量图、超声造影、超声弹性成像等,并阐明其机制。由此发现超声在早期评估与诊断糖尿病肾病中起到了重要的作用。Diabetic nephropathy is a specific complication of microangiopathy caused by chronic kidney disease, and it is a common cause of end-stage renal failure. The mechanism of diabetic nephropathy complication is very complex. At present, it has not been fully clarified. For its treatment, there is no specific treatment. Diabetes can bring a series of target organs such as heart, kidney, and fundus, and 40% of the diabetic patients will develop into chronic kidney disease, and then develop to end-stage renal disease. Therefore, early diagnosis and early detection of diabetic nephropathy should not be ignored. By studying the literature of ultrasonic diagnosis of diabetic nephropathy in recent years, the ultrasonic diagnostic methods of several kinds of diabetic nephropathy were enumerated, including ultrasonic backscatter integration, color Doppler flow imaging, color Doppler energy map, ultrasound contrast, ultrasound elastography and so on, and clarified the mechanism. It is found that ultrasound plays an important role in early diagnosis and diagnosis of diabetic nephropathy.
