乌司他丁联合大剂量氨溴索对重症肺炎患者血清心肌酶、降钙素原水平的影响

摘　　要：目的:探究乌司他丁联合大剂量氨溴索对重症肺炎患者血清心肌酶、降钙素原水平的影响。方法:随机将188例重症肺炎患者分为观察组(乌司他丁加氨溴索治疗)及对照组(氨溴索治疗),于治疗前、治疗2周后检测两组患者的血清炎症细胞因子降钙素原(PCT)及血清心肌酶,包括乳酸脱氢酶(LDH)、天冬氨酸转氨酶(AST)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、羟丁酸脱氢酶(α-HB-DH)水平差异;观察治疗前、后两组患者的肺功能指标如峰流速(PEF)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、第1秒用力呼气容积占用力肺活量比值(FEV1%)、最大呼气中段流量(MMF)水平等。结果:两组患者治疗2周后,血清炎症细胞因子PCT及血清心肌酶(LDH、AST、CK、CK-MB、α-HB-DH)水平均较治疗前降低(P <0. 05),且观察组患者低于对照组(P <0. 05);肺功能指标(PEF、FVC、FEV1、FEV1%、MMF)均较治疗前增高(P <0. 05),且观察组高于对照组(P <0. 05)。结论:乌司他丁联合大剂量氨溴索对于重症肺炎患者的病情有良好的控制作用,促进患者肺功能恢复。Objective:To explore the effects of ulinastatin combined with high dose of ambroxol on the levels of serum myocardial enzymes and procalcitonin in patients with severe pneumonia.Methods:188 cases of patients with severe pneumonia were divided into study group(ulinastatin and ambroxol,n=94)and control group(ambroxol,n=94)according to the random number table method.The levels of serum inflammatory cytokine procalcitonin(PCT)and serum myocardial enzymes,including lactate dehydrogenase(LDH),aspartate aminotransferase(AST),creatine kinase(CK),creatine kinase isoenzyme(CK-MB)and hydroxybutyrate dehydrogenase(α-HB-DH),were detected in the two groups before treatment and after 2w of treatment,and the levels of pulmonary function indicators,including peak flow rate(PEF),forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),ratio of forced expiratory volume in 1 second of FVC(FEV1%),and maximal mid-expiratory flow(MMF),were observed before and after treatment.Results:After 2w of treatment,the levels of serum inflammatory cytokine PCT and serum myocardial enzymes(LDH,AST,CK,CK-MB,α-HB-DH)were lower than those before treatment(P<0.05),and the levels in study group were lower than those in control group(P<0.05);the levels of pulmonary function indicators(PEF,FVC,FEV1,FEV1%,MMF)were higher than those before treatment(P<0.05),and the levels in study group were higher than those in control group(P<0.05).Conclusions:Ulinastatin combined with high dose of ambroxol has a good control of disease condition of patients with severe pneumonia,and promotes the recovery of pulmonary function.
