摘 要:目的探讨高分辨率彩超对早产儿颅内病变的诊断价值。方法选取我院2013年7月至2018年6月确诊为颅内病变的早产儿80例,进行高分辨率彩超检查及CT检查。比较高分辨率彩超与CT对颅内病变的检出情况。结果高分辨率彩超对颅内病变的诊断效能(灵敏度、特异度、阳性预测值及阴性预测值)均高于头颅CT (P <0.05)。高分辨率彩超对颅内病变的总检出率高于头颅CT (P <0.05)。高分辨率彩超对Ⅰ级颅内出血、缺氧缺血性脑病、脑室周围白质软化的阳性检出率均高于头颅CT(P <0.05)。结论高分辨率彩超对早产儿颅内病变尤其是对颅内出血及缺氧缺血性脑病的诊断价值较高。Objective To explore the diagnostic value of high-resolution color Doppler ultrasonography on craniocerebral diseases of premature infants.Methods 80 cases of premature infants diagnosed as craniocerebral diseases in our hospital from July 2013 and June 2018 were selected and given high-resolution color Doppler ultrasonography and CT.The detection on craniocerebral diseases was compared between high-resolution color Doppler ultrasonography and CT.Results The diagnostic efficacy(sensitivity,specificity,positive predictive value,and negative predictive value)on craniocerebral diseases of high-resolution color Doppler ultrasonography was higher than that of head CT(P<0.05).The total detection rate on craniocerebral diseases of high-resolution color Doppler ultrasonography was higher than that of head CT(P<0.05).The positive detection rates of gradeⅠintracranial hemorrhage,neonatal hypoxic-ischemic encephalopathy and periventricular leukomalacia in premature infants of high-resolution color Doppler ultrasonography were higher than those of head CT(P<0.05).Conclusions High-resolution color Doppler ultrasonography has higher diagnostic value in craniocerebral diseases of preterm infants,especially for intracranial hemorrhage and hypoxic-ischemic encephalopathy.
