摘  要:目的探讨中心静脉导管行胸腔闭式引流和高频振荡通气(HFOV)治疗新生儿气胸的临床疗效。方法选取2015年12月至2017年9月期间我院收治的170例气胸新生儿,根据治疗方法分为HFOV组和引流组各85例。引流组采用中心静脉导管行胸腔闭式引流治疗, HFOV组采用HFOV。分析比较两组患儿的呼吸机使用时间、胸片恢复正常时间、住院时间和治疗效果。结果引流组的呼吸机使用时间、胸片恢复正常时间均短于HFOV组,但住院时间长于HFOV组(P均<0.05)。HFOV组的痊愈率为97.65%,高于引流组的88.24%(P <0.05)。结论中心静脉导管行胸腔闭式引流和HFOV均能够有效改善气胸新生儿的血气指标和肺功能,各有优势。Objective To explore the clinical curative effect of closed thoracic drainage and high-frequency oscillatory ventilation(HFOV)of central venous catheter in the treatment of neonatal pneumothorax.Methods 170 cases of neonates with pneumothorax admitted to our hospital from December 2015 to September 2017 were selected and divided into HFOV group and drainage group,with 85 cases in each group.The drainage group was given closed thoracic drainage in central venous catheter,while the HFOV group received HFOV.The ventilator use time,time for chest radiograph back to normal,hospitalization time,and treatment effect were analyzed and compared between the two groups.Results The ventilator use time and time for chest radiograph back to normal of the drainage group were shorter than those of the HFOV group,but the hospitalization time was longer than that of the HFOV group(all P<0.05).The recovery rate of the HFOV group was 97.65%,higher than 88.24%of the drainage group(P<0.05).Conclusions Closed thoracic drainage and HFOV in central venous catheter can effectively improve the blood gas index and lung function of neonates with pneumothorax,which have their own advantages.
