摘  要:目的脓毒症表现为高分解代谢,可引起不能纠正的急性骨骼肌分解,右美托咪定在一定程度上减轻了脓毒症高分解代谢状态,因而具有潜在的改善急性骨骼肌分解的作用。文中探讨右美托咪定(DEX)对内毒素血症大鼠下丘脑神经肽及骨骼肌萎缩基因表达的影响。方法 36只雄性SD大鼠随机数字表法分为:空白对照组,模型组和干预组。利用腹腔注射脂多糖建立内毒素血症模型。干预24 h后,采用RT-qPCR方法检测大鼠下丘脑神经肽(POMC、CART、Ag RP、NPY)及骨骼肌MuRF-1、MAFbx mRNA的表达。结果空白对照组、模型组、干预组POMC mRNA表达分别为(0.51±0.22、1.65±0.32、0.84±0.36),CART mRNA表达分别为(0.44±0.28、1.87±0.43、0.98±0.26),Ag RPmRNA表达分别为(3.86±0.54、1.21±0.11、4.76±0.39),NPYmRNA表达分别为(0.74±0.31、1.05±0.17、0.87±0.22)。与空白对照组比较,模型组下丘脑食欲减退神经肽POMC和CART mRNA表达显著升高,Ag RP mRNA表达显着下降(P<0.05);与模型组比较,干预组下丘脑食欲减退神经肽POMC和CART mRNA表达明显下降,Ag RP mRNA表达明显升高(P <0.05)。与空白对照组比较,模型组和干预组MuRF-1和MAFbx mRNA表达显著升高(P<0.05);与模型组比较,干预组MuRF-1和MAFbx mRNA表达显著下降(P<0.05)。结论右美托咪定能调节内毒素血症大鼠下丘脑神经肽及骨骼肌肌肉萎缩基因的表达。Objective Sepsis is often accompanied by serious disorder of energy metabolism,which is characterized by high catabolism,leading to irreversible acute skeletal muscle decomposition.As a new sedative,dexmedetomidine can alleviate the hypercatabolism of sepsis to a certain extent,so it has the potential to improve acute skeletal muscle decomposition.The present work aims to investigate the effects of dexmedetomidine on the expressions of hypothalamus neuropeptides and skeletal muscle atrophy gene in endotoxemic rats. Methods Thirty-six male Sprague Dawley rats were randomly divided into three groups(n=12):control group(group CON),-model group(group LPS-CON)and intervention group(group LPS-DEX).The endotoxemic rat model was established by injecting lipopolysaccharide(LPS)intraperitoneally.After 24 hours intervention,the expressions of hypothalamic neuropeptide(POMC,CART,AgRP and NPY)mRNA as well as MuRF-1 and MAFbx mRNA in skeletal muscle in rats were detected by RT-PCR. Results Compared with control group,the expressions of POMC,CART,MuRF-1 and MAFbx mRNA were significantly up-regulated(P<0.05),while the expression of AgRP mRNA was significantly down-regulated in-model group(P<0.05).Compared with-model group,the expressions of POMC,CART,MuRF-1 and MAFbx mRNA were significantly down-regulated(P<0.05),while the expression of AgRP mRNA was significantly up-regulated in intervention group(P<0.05).Conclusion Dexmedetomidine could regulate the expressions of hypothalamus neuropeptides and skeletal muscle atrophy gene in endotoxemic rats.
