长春市就诊居民社区卫生服务评价的定性研究

摘　　要：目的:了解长春市某城区社区卫生服务中心(Community Health Service Center,CHC)就诊居民对社区卫生服务的评价及建议,为完善社区卫生服务提供针对性对策。方法:2016年11月-12月,立意选择长春市某城区5家CHC,对愿意接受访谈的就诊居民进行半结构式访谈,采用主题框架法对访谈内容进行分析。结果:CHC在卫生服务可及性方面发挥了重要作用,但仍存在优惠力度不足、不能满足居民用药需求、就医环境一般、诊疗水平不受患者信任、医务人员个性化服务提供不足等问题。居民对产生较少经济负担的卫生服务、方便舒适的卫生服务利用过程、高水平的诊疗服务以及个性化服务有较高需求。结论:提升社区卫生服务水平,政府需完善基本药物政策,扩大医保慢病补贴、探索慢病购药长处方政策,加大弱势人群医疗救济保障,鼓励CHC扩大药品储备,加大对CHC基础建设投入,加大CHC人力资源投入,探索医生多点执业的具体措施,建立健全以服务质量和健康结果为导向的考核和激励政策。Objective:To understand the comments and suggestions for community health service of Community Health Service Centers(CHC)of residents in Changchun,and to provide corresponding countermeasures for Community Health Service improvement.Methods:Semi-structured interviews were conducted with patients willing to accept interview in 5 CHCs selected by purposive sampling method in one district in Changchun during November to December in 2016.Theme framework method was used to analyze the interview.Results:CHC plays an important role in health services accessibility.However,insufficient preferential price,unmet medicine demand,imperfect medical environment,patients’distrust to diagnosis and treatment of CHC,undersupply of personalized services were still existing currently.At meanwhile,residents had high demand for health services that produce less economic burden,convenient and comfortable health service utilization process,high level of diagnosis and treatment services,and personalized services.Conclusion:To improve community health service,government needs to consummate the basic drug policy,to expand health care subsidies,to explore policy of long description of chronic medicine purchase,to provide more medical remedy protection to vulnerable person,to encourage CHC to expand pharmaceutical reserve,to increase investment in infrastructure and human resources in CHC,to explore doctors’multi-location-practice,and to establish and improve assessment and incentive policies oriented by service quality and health results of residents.
