山东省胰腺癌放疗设备现状调查及分析

摘　　要：目的:了解山东省胰腺癌放疗设备应用情况及人员配置,明确国产与进口放疗设备的适应证范围及质量控制情况。方法:选择山东省内11家不同等级医院(2家省级、9家地市级),回顾性分析各级医院胰腺癌放疗患者的临床资料及放疗数据,对放疗设备分布情况、剂量参数及放射治疗计划系统(treatment therapy planning system,TPS)执行能力、质量控制情况等进行调研。结果:省级医院直线加速器共13台,均为进口设备;地市级医院直线加速器共12台,以国产居多。进口设备剂量验证显示各参数稳定,计划适形度及加速器计划执行能力高;国产设备计划执行能力、靶区适形性及均匀性指数良好,但调强放疗(intensity modulated radiation,IMRT)时间长。11家医院能够完成直线加速器的定期测量和校准,但有3家地市级医院缺乏严格的质量控制规范。结论:省级医院以进口放疗设备居多,有完整的质量控制措施和程序,基本满足胰腺癌放疗的各项临床要求。而地市级医院放疗设备缺口较大,国产放疗设备能保证控制患者病情,但质量控制措施和程序不完善,且物理师数量和水平较省级医院存在一定差距,现阶段建议只承担晚期胰腺癌姑息性放疗。Objective To understand the application and personnel of the radiation therapy equipment for pancreatic cancer in Shandong province,and to make clear the indications and quality control of China-made and imported radiation therapy equipment.Methods Totally two grades of hospitals,including two provincial ones and nine municipal ones,had their pancreatic cancer patients analyzed retrospectively on radiation therapy data,and radiation therapy equipment survey was executed on the distribution,dosimetry parameters,executive capability of radiation therapy planning system,quality control and etc.Results The thirteen medical linear accelerators were all imported in the two provincial hospitals,and the twelve accelerators were mostly domestic in the nine municipal hospitals;dose verification showed the imported equipment had stable parameters and high planning conformity and accelerator executive capability,and the China-made ones behaved well in executive capability,target conformity and homogeneity index although its IMRT time was not so satisfactory;all the eleven hospitals carried out periodic measurement and calibration of the linear accelerator,while three municipal hospitals had no strict quality control specifications.Conclusion The provincial hospital meets the clinical requirements of pancreatic cancer therapy in equipment and quality control,while the municipal hospital is suggested to execute palliative irradiation of late pancreatic cancer patients due to its disadvantages in equipment quality control and medical physicist.
