优福宁胶囊联合胸腔镜下冷冻治疗结核性包裹性胸腔积液疗效及对氧化应激指标、PON1的影响

摘　　要：目的观察优福宁胶囊联合胸腔镜下冷冻治疗结核性包裹性胸腔积液疗效及对肺功能、氧化应激指标、血清对氧磷脂酶-1(PON1)的影响。方法将82例结核性包裹性胸腔积液患者随机分为观察组和对照组,每组41例。对照组患者给予常规抗结核及胸腔镜下冷冻治疗,观察组患者在对照组治疗基础上给予优福宁胶囊口服。检测2组治疗前及治疗3个月后肺功能指标[肺活量(FVC)、1 s用力呼气容积(FEV1)及呼气峰流速(PEF)]、氧化应激指标[谷胱甘肽过氧化物酶(GSH-Px)、总抗氧化能力(T-AOC)、超氧化物歧化酶(SOD)及总抗氧化态(TAS)]和治疗前、治疗1个月、治疗2个月、治疗3个月后血清PON1水平并进行比较,记录2组胸水吸收时间、住院时间及治疗3个月后胸膜厚度,统计2组治疗3个月后临床疗效及治疗期间出现的并发症和不良反应。结果治疗3个月后,2组患者FVC、FEV1、PEF、SOD、GSH-Px、T-AOC及TAS均明显增高(P均<0. 05),且观察组均明显高于对照组(P均<0. 05);观察组治疗1个月、2个月、3个月后血清PON1水平均明显高于本组治疗前与同期对照组(P均<0. 05),对照组则治疗2个月、3个月后明显高于治疗前(P均<0. 05);观察组胸水吸收时间及住院时间均明显短于对照组(P均<0. 05),治疗3个月后胸膜厚度明显低于对照组(P <0. 05);观察组总有效率明显高于对照组(P <0. 05)。治疗期间2组患者均未出现严重并发症及不良反应。结论优福宁胶囊联合胸腔镜下冷冻治疗结核性包裹性胸腔积液疗效更好,可明显改善肺功能,且安全性较高,可能与增强机体抗氧化应激能力有关。Objective It is to observe the effect of Youfuning Capsule combined with thoracoscopic cryoablation on tuberculous encapsulated pleural effusion and its effect on lung function,oxidative stress index and serum phospholipase-1(PON1).Methods Eighty-two patients with tuberculous pleural effusion were randomly divided into observation group and control group,with 41 cases in each group.The patients in the control group were given conventional anti-tuberculosis and thoracoscopic cryotherapy.The patients in the observation group were given Youfuning Capsules orally on the basis of the control group.Pulmonary function indicators[fluid capacity(FVC),1 second forced expiratory volume(FEV 1)and peak expiratory flow rate(PEF)],oxidative stress index[glutathione peroxidation enzyme(GSH-Px),total antioxidant capacity(T-AOC),superoxide dismutase(SOD)and total antioxidant state(TAS)]before and after 3 months of treatment,the levels of serum PON1 before treatment and after treastment for 1,2,3 months were detected and compared,the pleural absorption time,hospitalization time and pleural thickness after 3 months of treatment were recorded.The clinical efficacy was assessed and complications and adverse reactions during the treatment were recorded after 3 months.Results After 3 months of treatment,FVC,FEV 1,PEF,SOD,GSH-Px,T-AOC and TAS were significantly increased in both groups(P<0.05),and the increases in the observation group were more significant than the control group(P<0.05);after treatment for 1,2,3 months,serum PON1 level in the observation group was significantly higher than that before treatment and in the control group(P<0.05),the levels after treatment for 2,3 months were higher than that before treatment in the control group(P<0.05).The pleural water absorption time and hospitalization time of the observation group were significantly shorter than the control group(P<0.05),the pleural thickness was significantly lower after 3 months of treatment than the control group(P<0.05).The total effective rate of the observation
