益肾健脾通络汤对特发性膜性肾病患者尿蛋白及凝血功能的影响

摘　　要：目的观察益肾健脾通络汤对特发性膜性肾病患者尿蛋白及凝血功能的影响。方法将84例特发性膜性肾病患者随机分为2组,对照组42例给予常规西药治疗,观察组42例在对照组治疗基础上给予益肾健脾通络汤治疗,2组均连续治疗3个月。观察2组治疗后24 h尿蛋白定量(24 h UTP)、血清白蛋白(ALB)、凝血功能[凝血酶原时间(PT)、凝血酶时间(TT)、血小板计数(Plt)]及肾功能指标[血肌酐(SCr)、尿素氮(BUN)]改善情况,评估2组临床疗效。结果治疗后2组24 h UTP、SCr、BUN水平和Plt均显著降低(P均<0. 05),ALB水平均显著升高(P均<0. 05),PT、TT均显著延长(P均<0. 05),且治疗后观察组上述各指标改善情况均显著优于对照组(P均<0. 05);观察组治疗后总有效率为90. 5%,显著高于对照组的73. 8%(P <0. 05); 2组治疗期间均未出现严重不良反应。结论益肾健脾通络汤能够明显改善特发性膜性肾病患者凝血功能和肾功能,减少蛋白尿,提高血清白蛋白水平,疗效确切,安全可靠。Objective It is to observe the effects of Yishen Jianpi Tongluo Decoction on urinary protein and coagulation function in patients with idiopathic membranous nephropathy.Methods Eighty-four patients with idiopathic membranous nephropathy were randomly divided into two groups.42 patients in the control group were treated with conventional western medicine.42 patients in the observation group were treated with Yishen Jianpi Tongluo Decoction on the basis of the control group.Both groups were continuously treated for 3 months.The improvements of 24-hour urine protein quantitation(24 hUTP),serum albumin(ALB),coagulation function[prothrombin time(PT),thrombin time(TT),platelet count(Plt)]and renal function indexes[serum creatinine(SCr),urea nitrogen(BUN)]after treatment were observed in the two groups,and the clinical efficacy of the two groups was evaluated.Results After treatment,the levels of 24 hUTP,SCr,BUN and Plt were significantly decreased(P<0.05),ALB levels were significantly increased(P<0.05),and PT and TT were significantly prolonged in the two groups(P<0.05),the improvements of the above indicators in the observation group were significantly better than that of the control group(P<0.05);the total effective rate of the observation group was 90.5%,which was significantly higher than that of the control group(73.8%)(P<0.05);No serious adverse reactions occurred during the treatment period.Conclusion Yishen Jianpi Tongluo Decoction can significantly improve coagulation function and renal function,reduce proteinuria and increase serum albumin level in patients with idiopathic membranous nephropathy.The curative effect is exact,safe and reliable.
