摘 要:目的 :观察较低热能肠内营养支持对脑血管意外患者和肺部感染患者的影响,探讨更为合适且有效的肠内营养支持方法。方法 :收集采取低热量肠内营养支持的患者88例,其中,脑血管意外患者47例,肺部感染患者41例,比较两组患者在肠内营养支持前后血红蛋白(hemoglobin, Hb)、总蛋白(total protein, TP)、白蛋白(albumin, ALB)、前白蛋白(PreALB)和空腹血糖(glucose, Glu),观察两组患者住院天数和死亡人数的差异。结果:与营养支持前相比,仅肺部感染组HB显著降低(P <0.05);两组间比较,营养支持前TP、ALB对比有显著性差异(P <0.05);营养支持后,HB、TP和ALB对比有显著性差异(P <0.05)。两组患者住院天数和患者死亡率均无显著性差异。结论 :肺部感染患者对能量需求较高,应提高肠内营养的能量供给。针对不同患者,不同疾病,应优化营养支持方案,合理使用肠内营养制剂,深入研究并规范肠内营养在不同患者身上的应用。Objective:To observe the effect of enteral nutrition support with lower thermal energy on patients with cerebrovascular accident or pulmonary infection,and to explore more suitable and effective enteral nutrition support methods.Methods:Eighty-eight patients with cerebrovascular accident(n=47)or pulmonary infection(n=41)who received low calorie enteral nutrition support were collected.The changes of the levels of hemoglobin(HB),total protein(TP),albumin(ALB),prealbumin(pre-ALB)and fasting blood glucose(Glu)were compared between the two groups before and after enteral nutrition support,and the differences of the hospital stay and the death toll were observed.Results:Compared with before nutritional support,only pulmonary infection group had a significant decrease in HB(P<0.05).There were significant differences in TP and ALB levels between the two groups before nutritional support(P<0.05),however,there were significant differences in HB,TP and ALB levels after nutritional support(P<0.05).There were no significant differences in hospital stay and mortality between the two groups.Conclusion:The patients with pulmonary infection have higher energy requirements and their energy supply of enteral nutrition should be increased.For different patients and diseases,nutritional support programs should be optimized,enteral nutrition preparations should be rationally used,and the application of enteral nutrition in different patients should be further studied and standardized.
