摘 要:目的为了规范院后随访流程,打造院后一体化平台,实现患者从离院到家庭到再入院的闭环管理,并实现跨科室、跨医疗组的协作,提高患者医疗服务的安全性和医疗质量水平。方法系统采用B/S结构,结合移动APP的模式构建统一患者管理平台,通过采集患者的各种相关数据,为患者在家庭的监测和护理提供最佳的服务远程管理模式。结果通过云随访平台获取相关数据形成人群健康实时数据库,创建全程患者健康档案。对健康数据进行进一步地挖掘、分析后,给出医生的诊断建议,有效地缓解了医疗资源的紧张,并实现患者的院内外一体化健康教育管理。结论云随访平台获取量化数据,准确统计分析相关医疗信息,实现了随访的科学规范管理及科研项目的跟踪,促进医护患沟通,降低医患矛盾,并对疾病的治疗、预防和康复进行有效管理。Abstrac: Objective To standardize the post-hospital follow-up process and create the post-hospital integration platform, the closedloop management was realized for patients from leaving the hospital to family to readmission, and the collaboration between departments and medical teams was realized to improve the safety and medical quality of patients’ medical services. Methods The unified patient management platform was built by using B/S structure and combining with the mode of mobile APP. Then, the best service remote management mode for patients’ monitoring and nursing in their family was provided by collecting various related data of patients. Results Relevant data were obtained through cloud follow-up platform to form crowd health real-time database and completed patient health archives were created. After further mining and analysis of health data, the doctors’ diagnosis suggestions were given to patients, which effectively alleviated the shortage of medical resources and realized the integrated health education management of patients in and out of the hospital. Conclusion Cloud follow up platform can get quantized data, analyze the related medical information accurately, realize scientific and standardized management of follow-up and tracking of research projects. Contradictions between doctors and patients are reduced by promoting communication between them. The treatment, prevention and rehabilitation of the disease is effectively managed. All above benefits from the application of cloud follow-up platform.
