摘　　要：目的分析可膨胀交锁髓内钉(Fixion IL)治疗成年浮膝损伤的临床效果。方法回顾性分析2011年1月至2017年12月在我科采用Fixion IL进行治疗的36例浮膝损伤患者的临床资料,其中Fraser分型Ⅰ型16例,Ⅱa型16例,Ⅱb型4例。分析手术时间、术中出血量、术中透视次数、切口愈合情况、骨折愈合情况、膝关节功能及术后并发症。结果获得随访36例,随访24～48周,平均(32.4±11.3)周,骨折均愈合。最后一次随访时获得HSS评分为60～96分,平均(85.6±10.3)分,评分优秀。并发症6例,包括下肢静脉血栓3例,关节僵硬2例,股骨骨不连1例。结论 Fixion IL具有手术时间短、术中出血少、固定应力分布均匀、末端无需锁定等优点,是治疗浮膝损伤的有效方法。Objective To analyze the clinical effect of expandable intramedullary nailing(Fixion IL)in the treatment of adult floating knee injury.Methods The clinical data of 36 patients with floating knee injuries treated with Fixion IL in our department from January 2011 to December 2017 were retrospectively analyzed.Fraser classification included 16 cases of typeⅠ,16 cases of typeⅡa and 4 cases of typeⅡb.The operation time,intraoperative bleeding amount,intraoperative fluoroscopy times,incision healing,fracture healing,knee joint function and postoperative complications were analyzed.Results Thirty-six cases were followed up for 24-48 weeks,with an average of(32.4±11.3)weeks.All fractures healed.At the last follow-up,the HSS score was 60-96,with an average of(85.6±10.3)and it was excellent.The six complications included venous thrombosis of lower extremity in 3 cases,stiffness of joint in 2 cases and nonunion of femur in 1 case.Conclusion Fixion IL has the advantages of short operation time,less intraoperative bleeding,uniform distribution of fixed stress and no need of locking at the distal end.It is an effective method to treat floating knee injury.
