外侧锁定加压钢板内固定术治疗闭合性胫骨下端骨折的临床效果

摘　　要：目的观察外侧锁定加压钢板内固定术治疗闭合性胫骨下端骨折的效果。方法选择我院收治的闭合性胫骨下端骨折患者90例为研究对象,根据手术方法分为观察组(行闭合复位胫前外侧锁定加压钢板内固定术)和对照组(行闭合复位胫前内侧锁定加压钢板内固定术),各45例。比较两组患者的手术指标、临床效果及手术前后血清IL-6及TNF-α水平。结果观察组患者的功能锻炼开始时间、手术时间、住院时间和骨折愈合时间均短于对照组,术中出血量少于对照组(P<0.05)。观察组患者的治疗总有效率明显高于对照组(P<0.05)。术后,两组患者的血清IL-6和TNF-α水平均降低,且观察组低于对照组(P<0.05)。结论外侧锁定加压钢板内固定术治疗闭合性胫骨下端骨折效果明显,值得推广。Objective To observe the effect of anterolateral locking compression plate fixation on closed distal tibial fractures. Methods Ninety patients with closed distal tibial fractures admitted in our hospital were selected as the research objects. According to the operation methods, the patients were divided into observation group(closed reduction and anterolateral locking compression plate fixation) and contr ol group(closed reduction and anterior tibial medial locking compression plate fixation), with 45 cases in each group. The operative indexes, clinical effects and levels of serum IL-6 and TNF-α in the two groups were compared. Results The start time of functional exercise, operation time, hospitalization time and fracture healing time of the observation group were shorter than those of the control group, and the intraoperative blood loss amount was less than that of the contro l group(P <0.05). The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05). After operation, the serum levels of IL-6 and TNF-α of the two groups decreased, and those of the observation group were lower than the control group(P<0.05). Conclusion Anterolateral locking compression plate fixation on closed distal tibial fractures has an obvious effect, and it is worth popularizing.
