摘  要:目的探讨在环枢椎骨折脱位情况下,进行Halo-Vest支具的具体效能与并发症及临床效果。方法 16例上颈椎骨折脱位患者,釆用Halo-Vest支具保守治疗,于术后6周、16周门诊复查,CT提示骨折愈合(骨小梁通过骨折线,没有假关节形成)后拆除Halo-Vest支具,拆除支具后行颈椎过伸过屈位片评估颈椎稳定性。结果在后期拆除支具后钉道口均良好愈合,无颅骨骨髓炎以及颅内脓肿、癫痫发作等罕见相关并发症出现。结论 Halo支具固定是治疗上颈椎骨折脱位的有效选择,可显著降低骨折移位发生概率,降低骨折固定期间的微动,有利于骨折早期愈合;在术前急救搬运,有开放手术禁忌者,采用这项传统固定方式效果最佳。Objective To discuss the effectiveness,complications and clinical effects of the Halo-Vest brace under the condition of annular axial fracture and dislocation.Methods 16 patients with upper cervical spine fractures and dislocations were treated with Halo-Vest brace conservatively.The patients were reviewed 6 weeks and 16 weeks after operation.Ct suggests removal of halo-vest anchorage after fracture healing,and the stability of the cervical spine was evaluated by displacing the brace.Result The patients in this group were well-healed after the dismantling of the brace at the later stage.No complications such as osteomyelitis of the skull,intracranial abscess and epilepsy occurred.Conclusion It is an effective choice for the treatment of fracture and dislocation of upper cervical spine by using the Halo brace fixation.It can significantly reduce the probability of fracture displacement,reduce the fretting during the fixation period of fracture and facilitate the early healing of fracture.For the patients with first aid handling before surgery or with open surgery contraindications,this traditional fixing method works best.
