摘  要:目的观察低分子量肝素对大鼠重症急性胰腺炎(SAP)胰腺和肺组织病理变化及血清TNF-a和IL-6的影响作用。方法选用Wistar大鼠随机分为模型组和低分子量肝素组(简称肝素组),分别检测术后6 h、12 h和24 h胰腺和肺组织形态学及血清TNF-a和IL-6的变化。结果胰腺与肺组织形态学变化:胰腺水肿、炎细胞浸润、出血和坏死以12~24 h最重;术后6 h肺实质和肺泡腔已有大量炎细胞浸润,出血以12 h最严重,24 h肺泡坏死并融合出现大量空泡,胰腺和肺组织病理变化肝素组比模型组明显减轻,胰腺和肺组织病理评分肝素组明显低于模型组(P<0.001);血清TNF-α和IL-6的水平肝素组显著降低,与模型组比较(P<0.001)。结论低分子量肝素抑制TNF-α和IL-6水平,早期干预可减轻SAP所致的胰腺和肺组织损伤。Objective To observe the effects of low molecular weight heparin(LMWH)on pancreatic and lung histopathological changes and serum TNF-αand IL-6 in rats with severe acute pancreatitis(SAP).Methods Wistar rats were divided into the model group and the low molecular weight heparin group(heparin group).The changes of the morphology of pancreas and lung tissue,serum TNF-a and IL-6 were detected 6h,12h and 24h after the surgery respectively.Results Morphological changes of the pancreas and lung tissue:Pancreatic edema,inflammatory cell infiltration,hemorrhage and necrosis were the most severe in 12-24h.After 6h of the surgery,the pulmonary parenchyma and alveolar cavity has been infiltrated by a large number of inflammatory cells,the hemorrhage is most severe after 12h.After alveolar necrosis and fusion at 24h,a large number of vacuoles appeared.The pathological changes of pancreas and lung tissue in heparin group were significantly less than those in the model group,and the pathological score of pancreas and lung tissue in heparin group was significantly lower than that in the model group(p<0.001).Compared with the model group(P<0.001),the levels of serum TNF-αand IL-6 in the heparin group was significantly lower(p<0.001).Conclusion Low molecular weight heparin inhibits TNF-αand IL-6 levels and early intervention can alleviate SAP's pancreatic and lung tissue damage.
