血浆可溶性髓样细胞触发受体-1在儿童急性细菌感染腹泻中的诊断价值

摘　　要：目的探讨血浆可溶性髓样细胞触发受体-1(sTREM-1)在细菌感染引起的儿童急性腹泻诊断上的价值。方法收集在我院传染科住院的急性腹泻患儿80例,分为细菌腹泻组和非细菌腹泻组,选取30例年龄和性别匹配的健康婴儿作为对照组,比较3组的一般资料、粪便特征,ELISA法测定3组C反应蛋白(CRP)、s TREM-1浓度,受试者工作曲线(ROC)测定比较sTREM-1和CRP截断值、敏感度、特异度和曲线下面积(AUC)。结果细菌腹泻组sTREM-1较非细菌腹泻组和对照组显著增加[(26.37±16.82)ng/mL vs.(7.23±6.42)ng/mL vs.(6.74±5.65)ng/mL,P <0.05],细菌腹泻组CRP较非细菌腹泻组和对照组显著增加[(104.50±25.60)mg/L vs.(29.57±20.35)mg/L vs.(3.64±1.18)mg/L,P <0.05]。ROC曲线示,sTREM-1截断值为14.5 ng/mL时,敏感度为93.33%,特异度为90.33%,AUC为0.943;CRP的截断值为46 mg/L时,敏感度为94%,特异度为87%,AUC为0.920。结论 sTREM-1在儿童急性细菌感染性腹泻的诊断中具有较高的鉴别效能,可作为细菌性腹泻的诊断标志物。Objective To investigate the diagnostic value of soluble triggering receptor expressed on my-eloid cell-1(sTREM-1)in the diagnosis of children with acute diarrhea caused by bacterial infection.Methods Eighty children diagnosed with diarrhea were enrolled in this study,who were divided into bacterial infectious diarrhea group and non bacterial infectious diarrhea group.Thirty ageand sexmatched healthy children were enrolled in the control group.The general information and fecal characteristics were compared among these three groups.The serum C-relation protein(CRP),sTREM-1 concentration was measured by ELISA.The receiver oper-ating characteristic curve(ROC)was used to test the cutoff value,area under curve,sensitivity and specificity of sTREM-1 and CRP.Results Serum sTREM-1 level ofpatients in bacterial infectious diarrhea groupwas significantly higher than that in non bacterial infectious diarrhea group and in the control group[(26.37±16.82)ng/mL vs.(7.23±6.42)ng/mL vs.(6.74±5.65)ng/mL,P<0.05].Serum CRP level in bacterial infectious diarrhea group was significantly higher than that in non bacterial infectious diarrhea group and in the control group[(104.50±25.60)mg/L vs.(29.57±20.35)mg/L vs.(3.64±1.18)mg/L,P<0.05].ROC results showed that the optimum cut-off value of sTREM-1 was 14.5 ng/mL,sensitivity was 93.33%,specificity was 90.33%and AUC was 0.943.The optimum cut-off value of CRP was 46 mg/L,sensitivity was 94%,specificity was 87%and AUC was 0.920.Conclusions Serum sTREM-1 is highly effective in the diagnosis of children with acute bacterial infectious diar-rhea,which can be used as a diagnostic marker for bacterial diarrhea.
