预防性清扫阴茎癌患者双腹股沟淋巴结疗效观察

摘　　要：目的探讨阴茎癌手术同期行改良根治性双腹股沟淋巴结清扫术对腹股沟淋巴结临床阴性患者的安全性与可行性。方法回顾性分析11例双腹股沟淋巴结临床阴性阴茎癌患者的临床资料,患者术前活检均提示中-高分化鳞状细胞癌,均在阴茎癌原发病灶手术同时行双腹股沟淋巴结改良清扫术。结果 11例患者共22侧创口,无切口感染,局部皮下创口出现淋巴积液7侧次(31. 8%),经换药后均痊愈。术后住院10～21 d,每侧切除淋巴结6～13枚,临床腹股沟淋巴结阴性患者行清扫术提示淋巴结转移阳性率27. 3%;术后平均随诊3年,患者均未见再发淋巴结转移,无死亡病例。结论阴茎癌手术同期行改良根治性双腹股沟淋巴结清扫术安全可行,在无严重手术并发症的前提下可早期发现隐匿性转移病灶,并可降低患者术后腹股沟淋巴结转移复发率。Objective To investigate the feasibility and safety of modified inguinal lymphadennectomy for penile cancer with negative manifestation of lymph nodes.Methods The data of 11 penile cancer patients with negative manifestation of lymph nodes were reviewed,and preoperative biopsy of all the patients revealed medium-to-high differentiated squamous cell carcinoma,and all underwent surgery combined with modified inguinal lymphadennectomy.Results There were 22 incisions in these patients,without wound infection.There were 7 cases(31.8%)had subcutaneous exudation,and they were recovered after medicine changing.There were 6~13 lymph nodes dissected.Patients with clinical inguinal lymph node negative examination showed positive metastasis rate of lymph node(27.3%).There were no metastasis patients and death during 3-year follow up.Conclusions Penile surgery combined with simultaneously modified radical inguinal lymphadenectomy is feasible and safe,which can detect occult metastatic lesions when there were no severe complications,and decrease its re-occurrence rate.
