摘　　要：目的探讨日记式康复指导对人工全髋关节置换术后患者功能锻炼依从性的影响。方法将180例行单侧人工全髋关节置换术患者,分为两组各90例,对照组术后实施常规的功能锻炼,干预组给予日记式康复指导,即由医护联合负责制定每日功能锻炼计划,患者每次完成功能锻炼后,记录实际完成的项目、时间、不适等,护士长或护理组长评价功能锻炼的依从性,及时给予指导、追踪评价。结果干预组术后住院期间功能锻炼依从性、功能锻炼指导满意率、术后髋关节Harris评分显著高于对照组(均P<0.01)。结论日记式康复指导可提高人工全髋关节置换术后患者功能锻炼依从性,从而改善髋关节功能,提高患者满意度。Objective To investigate the effect of diary-style rehabilitation guidance on rehabilitation compliance in patients after total hip arthroplasty.Methods A total of 180 patients undergoing unilateral total hip arthroplasty were divided into two groups of 90 cases each.The control group received routine functional rehabilitation exercise,and the intervention group was given diary-style rehabilitation guidance:the doctors and nurses worked together to formulate training plans for each patient;after each session,the patients were asked to document the actual exercises they had completed,and the length of,and any discomfort related to the exercises;the head nurse or the nursing team leaders evaluate patients′compliance,and gave prompt instructions regarding the training.Results The compliance to functional exercise,satisfaction with functional exercise education,and the postoperative Harris Hip Scores in the intervention group,were significantly higher than those of the control group during hospitalization(P<0.01 for all).Conclusion Diary-style rehabilitation guidance could mobilize patients′compliance to functional exercise after total hip arthroplasty,thereby improve function of the hip joint and patient satisfaction.
